Teleodontologia na formação e no cuidado em saúde : análise de cirurgiões-dentistas da atenção primária no contexto da pandemia de covid-19

Abstract

INTRODUÇÃO: Teleodontologia é o uso de tecnologias de informação na formação e no cuidado em Odontologia. A pandemia de COVID-19 impulsionou o seu uso. OBJETIVO: Analisar, a partir da perspectiva de cirurgiões-dentistas (CDs) que atuam na Atenção Primária à Saúde (APS), o uso de Teleodontologia na formação e no cuidado em saúde, no contexto da pandemia de COVID-19, em municípios da 15ª Coordenadoria Regional de Saúde, Rio Grande do Sul (CRS/RS). METODOLOGIA: Estudo de delineamento observacional transversal. Os participantes foram CDs que atuavam na APS e ocupavam a representação pela área da Saúde Bucal em cada um dos 26 municípios que compõem a 15ª CRS/RS (n=26). A coleta de dados foi realizada pelo preenchimento de instrumento de pesquisa semiestruturado (45 questões), online, autoaplicável, pré-testado, entre dezembro de 2021 e janeiro de 2022. Questões objetivas (fechadas) foram analisadas pela estatística descritiva no software IBM® SPSS® Statistics e as abertas foram interpretadas pela análise temática de conteúdo, com o apoio do software Visual Qualitative Data Analysis (ATLAS.ti). A pesquisa foi aprovada por Comitê de Ética em Pesquisa. RESULTADOS: Os CDs participantes do estudo eram, em sua maioria, mulheres, de 25-44 anos, que haviam concluído a graduação entre 2011-2020, em instituição de ensino privada e realizado especialização nas áreas de Saúde Coletiva, Endodontia e Ortodontia. Eram estatutários e atuavam no Sistema Único de Saúde (SUS) de 10 a 20 anos e há mais de 20 anos (23,1%). 69,2% não recebiam estímulo financeiro para atividades de atualização profissional. Trabalhavam em municípios com população de até 5.000 habitantes (61,5%), com uma Equipe de Saúde Bucal (42,3%). Estes CDs não conheciam/não tinham lido a Resolução 226/2020 do Conselho Federal de Odontologia que trata da Teleodontologia (65,4%) e tinham estrutura para assistir/participar de atividades online no trabalho (96,2%) e em casa (100%). Na formação, 61,5% dos CDs não realizou nenhum curso/palestra/outra atividade de educação online, antes e durante o período da pandêmico, porém, mais de 80% acredita que são atividades que qualificam sua formação e trabalho. No cuidado em saúde, 57,7% conhecem e 46,2% já utilizaram o TelessaúdeRS (consultoria e cursos), 23,1% conhecem e 11,5% utilizaram o EstomatoNet e 26,9% já fez cursos UNA-SUS. As ações de Teleodontologia incluíram a orientação e/ou monitoramento para os pacientes por ligações telefônicas, WhatsApp e redes sociais. Com a pandemia, ações de orientação/monitoramento aumentaram de 26,9% para 38,5% e de teleagendamento de 50% para 69,2%. Na tele-educação, desafios/barreiras foram relacionados a dificuldades com as tecnologias, organização do tempo, necessidade de estar em ambiente privado, desinformação sobre a existência de cursos, por serem atividades consideradas mais cansativas, que não possibilitam a interação e as práticas, e a preferência por cursos presenciais. No cuidado, destacou-se o desconhecimento da Resolução que trata da Teleodontologia e as limitações do registro das ações realizadas na APS. PRODUTO: Foi desenvolvido o Produto de Comunicação Linha de Pesquisa ‘Processos de Ensino na Saúde’ – Vídeo Teleodontologia na APS/SUS para Equipes de Saúde Bucal –, aliando resultados da pesquisa com evidências da literatura sobre o tema. CONCLUSÃO: O estudo contribuiu para o entendimento da utilização da Teleodontologia na APS no cenário pré e pandêmico nos municípios da 15ª CRS/RS, mostrando potencialidades e desafios. A Teleodontologia foi reconhecida pelos CDs como uma ferramenta complementar importante na formação e no cuidado em saúde, podendo aumentar o acesso aos serviços. Estudos de abrangência estadual e nacional sobre a Teleodontologia no SUS são recomendados.

INTRODUCTION: Teledentistry is the use of information technologies in training and care in Dentistry. The COVID-19 pandemic has boosted its use. OBJECTIVE: To analyze, from the perspective of dentists (CDs) working in Primary Health Care (PHC), the use of Teledentistry in health training and care, in the context of the COVID-19 pandemic, in cities of the 15th Regional Health Coordination, Rio Grande do Sul (CRS/RS). METHODOLOGY: Cross-sectional observational design study. The participants were CDs who worked in PHC and occupied the representation by the Oral Health area in each of the 26 cities that make up the 15th CRS/RS (n=26). Data collection was performed by completing a semi-structured research instrument (45 questions), online, self-applicable, pre-tested, between December 2021 and January 2022. Objective (closed) questions were analyzed by descriptive statistics in the IBM® SPSS® Statistics Software and the open ones, by thematic content analysis, with the support of Visual Qualitative Data Analysis (ATLAS.ti) Software. The research was approved by the Research Ethics Committee. RESULTS: The CDs participating in the study were mostly women, 25-44 years old, who had completed their graduation between 2011-2020, in a private universitities and performed specialization in the areas of Collective Health, Endodontics and Orthodontics. They were statutory and worked in the Unified Health System (SUS) for 10 to 20 years and for more than 20 years (23.1%). 69.2% did not receive financial stimulus for professional updating activities. They worked in cities with a population up to 5,000 (61.5%), with an Oral Health Team (42.3%). These CDs did not know/had not read Resolution 226/2020 of the Federal Council of Dentistry that deals with Teledentistry (65.4%) and had the structure to assist/participate in online activities at work (96.2%) and at home (100%). In the training, 61.5% of the CDs did not perform any course/lecture/other online education activity, before and during the pandemic period, but more than 80% believe that they are activities that qualify their training and work. In health care, 57.7% knew and 46.2% already used the TelessaúdeRS (consulting and courses), 23.1% knew and 11.5% already used EstomatoNet and 26.9% did UNA-SUS courses. Teledentistry's actions included guidance and/or monitoring for patients by phone calls, WhatsApp and social networks. With the pandemic, guidance/monitoring actions increased from 26.9% to 38.5% and telescheduling from 50% to 69.2%. In tele-education, challenges/barriers were related to difficulties with technologies, time organization, need to be in a private environment, misinformation about the existence of courses, because they are activities considered more tiring, which do not allow interaction and practices, and preference for face-to-face courses. In the care, we highlight the ignorance of the Resolution that deals with Teledentistry and the limitations of recording the actions performed in PHC. PRODUCT: The Communication Product Research Line 'Teaching Processes in Health' was developed – Video Teledentistry in APS/SUS for Oral Health Teams – combining research results with evidence from the literature on the subject. CONCLUSION: The study contributed to the understanding of the use of Teledentistry in PHC in the pre- and pandemic scenario in the cities of the 15th CRS/RS, showing potentialities and challenges. Teledentistry was recognized by CDs as an important complementary tool in health education and care, and may increase access to services. State and national studies on Teledentistry in the SUS are recommended.
