A resistência antimicrobiana é um desafio atual à saúde pública. Agentes antimicrobianos são indispensáveis no controle de infecções bacterianas, não só em seres humanos, mas também em animais e plantas. A pressão seletiva imposta pela utilização sistemática de um antimicrobiano pode selecionar cepas com algum mecanismo de resistência e estas, disseminarem-se pelo ambiente. Muitas teorias e controvérsias existem a respeito da disseminação da resistência entre animais e humanos, além disso, a prevalência de genes que conferem resistência às cefalosporinas de espectro estendido, carbapenêmicos e colistina, em bactérias zoonóticas comensais é pouco conhecida. Este trabalho avaliou a prevalência dos principais genes de resistência de importância clínica (blaIMP -type, blaVIM -type, blaNDM-1, blaKPC -type, blaGES -type, blaOXA-48, blaTEM, blaSHV, blaCTX-M e mcr-1) em isolados de E. coli, originados de aves de produção. Foram coletados swab cloacal de frangos, em um abatedouro frigorífico localizado no Rio Grande do Sul. Foram obtidos 343 isolados de E. coli que cresceram em presença de ceftazidima. Entretanto, 57 isolados foram positivos para os genes que conferem resistência às cefalosporinas: 3 blaSHV, 18 blaCTX-M, 30 blaTEM e 6 para blaCTX-M e blaTEM concomitantemente Destes, 25 isolados foram positivos no teste fenotípico para pesquisa de ESBL. De acordo com o perfil de susceptibilidade aos antibióticos, 56 (98%) isolados foram considerados multirresistentes. Quanto à pesquisa do gene mcr-1, 10 isolados foram positivos (2,9%), sendo todos multirresistentes. Destes, 8 obtiveram valor de CIM para polimixina de 2mg/L, os outros dois isolados obtiveram CIM 0.25mg/L e 1mg/L. A tipagem molecular realizada por PFGE demonstrou que 5 isolados do mesmo lote foram clonalmente relacionados, enquanto outros 5 não tiveram relação clonal. A tipagem molecular realizada in silico a partir do sequenciamento completo do genoma bacteriano de 4 isolados positivos para o gene mcr-1 revelou a presença de 3 STs: ST38, ST58 e ST2491. A ST38 é bastante disseminada e associada com infecções em humanos. Diante da emergência da propagação do gene mcr-1 a partir de bactérias comensais de animais, torna-se crucial a implementação de práticas interdisciplinares no controle do uso de antimicrobianos na medicina veterinária, humana e no ambiente, evitando a disseminação da resistência e o esgotamento das opções terapêuticas. Alternativas ao uso indiscriminado dos antibióticos na produção animal junto a ações que procurem diminuir o potencial reservatório ambiental destes genes, devem ser implementadas. ...

Antimicrobial resistance is a current public health challenge. Antimicrobial agents are essential in the control of bacterial infections, not only in humans, but also in animals and plants. The pressure selection imposed by the systematic use of an antimicrobial, selects strains that harbor some resistance mechanism. The antibiotic resistance spread among animals and humans is controversial, furthermore, the prevalence of genes related to resistance to extended-spectrum cephalosporins, carbapenems and colistin in zoonotic commensal bacteria is not completely known. This study evaluated the prevalence of important clinical resistance genes (blaIMP -type, blaVIM -type, blaNDM-1, blaKPC -type, blaGES -type, blaOXA-48, blaTEM, blaSHV, blaCTX-M and mcr-1) in E. coli isolates originated from poultry production. Cloacal swabs were collected from chickens in a slaughterhouse located in Rio Grande do Sul. A total of 343 E. coli isolates were grown in the presence of ceftazidime. Genes encoding resistance to carbapenems, were not detected. However, 57 isolates were positive to cephalosporins resistance genes: 3 blaSHV, 18 blaCTX-M, 30 blaTEM and six isolates were co-producers of blaCTX-M and blaTEM. Phenotypic test for confirmation of ESBL, were positive for 25 isolates. According to the profile of susceptibility to antibiotics, 56 isolates were considered multiresistant (98%). Regarding the mcr-1 gene investigation, 10 isolates were positive, all of them multiresistant. The polymixin MIC of 8 isolates was 2 mg/L, the other two isolates presented MIC = 0.25mg/L and MIC = 1 mg/L. The molecular typing analysis, performed by PFGE, showed that 5 isolates from the same batch were clonally related, while another 5 were not related. Molecular typing performed in silico from complete sequencing of the bacterial genome of 4 mcr-1 positive isolates revealed the presence of 3 sequences type: ST38, ST58 e ST2491. With the present data in our hands and the emergence of mcr-1 gene, detected in commensal bacteria, with animal origins, it is crucial to implement interdisciplinary practices, to control the use of antimicrobials in veterinary medicine, human and the environment, avoiding the spread of resistance and depletion of therapeutic options. The indiscriminate use of antibiotics in animal production should be re-consider, as well as actions aiming to reduce the potential environmental reservoir of resistance genes, in order to prevent global spread. ...
