Somatosensory pathways of genital hedonic tactile sense : a systematic review with potential clinical applications

Abstract

Introduction: the study of the tactile subcategory of genital hedonic somesthesia has long been neglected when compared to the other somatosensory components, despite its relevance to the understanding of the human neurophysiology and the sexual dysfunction secondary to neurological disorders. Structured anatomical and functional circuitry models are yet to be elaborated. Purpose: this paper aims to topographically describe the components of afferent pathways related to genital hedonic somesthesia and to cite some of its potential clinical applications. Methods: a structured search strategy was applied using Pubmed, Embase and Web of Science databases, and studies contemplating the anatomical and functional aspects of the previously mentioned afferent pathways in humans, rats and mice were selected and critically analyzed. Results: the current data supports the elaboration of a topographically organized circuitry model, from terminal nerve endings to cortical structures, that facilitates our understanding of the afferent pathways related to the hedonic tactile sense, concomitantly identifying the sites that still require greater clarification, and elaborating on examples on how this knowledge is or could be applied clinically. Conclusions: providing structured information on this still neglected topic should not only allow for greater neuroscientific understanding of one of the most basic aspects of human behaviour, but also to improve the quality of clinical approaches to sexual dysfunction.

Introdução: o estudo da subcategoria tátil da somestesia hedônica genital é negligenciado frente aos outros componentes somatossensoriais a despeito de sua relevância para o entendimento da neurofisiologia humana e da disfunção sexual secundária a desordens neurológicas. Modelos funcionais e anatômicos destes circuitos ainda precisam ser elaborados. Motivo: esse estudo objetiva a descrição topográfica dos componentes das vias aferentes relacionadas à somestesia genital hedônica, bem como citar algumas de suas aplicações clínicas potenciais. Métodos: uma estratégia busca estruturada foi aplicada às databases do Pubmed, Embase e Web of Science, e estudos contemplando os aspectos anatomofuncionais das vias aferentes previamente mencionadas em humanos, ratos e camundongos foram selecionados e analisados criticamente. Resultados: os dados disponíveis dão suporte à elaboração de um modelo de circuitaria topograficamente organizado, das terminações nervosas até as estruturas corticais, que facilita nosso entendimento das vias aferentes relacionadas ao tato hedônico, concomitantemente identificando sítios que ainda requerem maiores esclarecimentos, e elaborando sobre exemplos de como este conhecimento é ou poderia ser aplicado clinicamente. Conclusões: prover informação estruturada neste tópico ainda negligenciado deve permitir um melhor entendimento neurocientífico de um dos aspectos mais básicos do comportamento humano, e também melhorar a qualidade das abordagens clínicas à disfunção sexual.
