Verificação das dimensões dentárias verticais no início e ao final do tratamento ortodôntico corretivo em casos tratados com e sem extração de pré-molares

Abstract

Características verticais da oclusão como a curva de Spee e o overbite têm uma excepcional importância para o planejamento de um tratamento ortodôntico. A alteração dessas medidas no decorrer do tratamento ortodôntico envolve outros fatores e requer conhecimento por parte do ortodontista para o manejo das condições clínicas das maloclusões. O objetivo deste trabalho é avaliar as dimensões verticais dentárias em modelos de gesso de 40 prontuários de pacientes da clínica do curso de Especialização em Ortodontia da Faculdade de Odontologia da Universidade Federal do Rio Grande do Sul, submetidos à tratamento ortodôntico com e sem extrações de pré-molares, ao início e ao término do tratamento, mensurando as alterações da curva de Spee e da sobremordida. As medidas foram realizadas nos modelos gesso a utilização de uma placa de acrílico e um paquímetro digital. Os valores obtidos ao final (T2) do tratamento foram comparados aos valores iniciais (T1). As variáveis curva de Spee e overbite também foram correlacionadas com T1 e T2. A análise dos resultados revelou que: 1) existem alterações significativas entre os valores de curva de Spee final e inicial no grupo com extrações, sendo a relação de overbite inicial com overbite final não significativa. 2) existem alterações significativas entre os valores de curva de Spee inicial e final, e entre overbite inicial e final no grupo sem extrações. 3) existe uma correlação direta positiva moderada entre valores de overbite inicial em valores de overbite final, sendo as demais correlações não significativas. Os resultados sugerem que apenas o overbite não sofre alterações significativas ao final do tratamento ortodôntico em pacientes cujo plano de tratamento envolve extrações dentárias, e que o valor do overbite inicial está associado com o valor do overbite final.

Vertical features of occlusion like the curve of Spee and overbite, have an exceptional importance to the planning of orthodontic treatment. Changing these measurements in the course of orthodontic treatment involves other factors and requires the knowledge of the orthodontist to manage the clinical condition of malocclusion. The purpose of this research is to evaluate the dental vertical dimensions in plaster models, at the beginning and at the end of treatment, by considering the medical records of 40 patients treated in the Orthodontic Specialization Course at the School of Dentistry of the Rio Grande do Sul Federal University. Patients were treated with and without premolar extractions, measuring the changes of the curve of Spee and overbite. Measurements were taken from plaster models with the utilization of an acrylic plate and a digital caliper. The values obtained at the end (T2) of treatment were compared to baseline values (T1). The variable curve of Spee and overbite were also correlated with T1 e T2. The results showed that: 1) there are significant alterations of the curve of Spee in the group with extractions, but the relation of the initial and final overbite is non-significant; 2) there are significant alterations between the values of initial and final curves of Spee and between the initial and final overbite in the non extraction group; 3) there is a moderately direct positive correlation between the values of initial overbite and the values of overbite at the end, however the remaining correlations are non-significant. The results suggest that only overbite does not change significantly at the end of orthodontic treatment in patients whose treatment plans involved extractions, and the values of the initial overbite can are associated with final values of overbite.
