A Klebsiella pneumoniae é uma bactéria bacilo gram-negativo da ordem Enterobacterales descrita inicialmente em 1882 por Friedlander (1). Essa cepa, usualmente referida como “clássica” - cKP, ao longo dos anos, se estabeleceu como agente etiológico de grande importância, dado sua capacidade de adquirir e acumular mecanismos de resistência. Em meados dos anos 80, novos relatos de caso na Ásia descreveram a emergência de uma nova cepa, hipervirulenta – hvKP, com características fenotípicas distintas e comportamento mais agressivo no hospedeiro, especialmente com sua capacidade de disseminação hematogênica mesmo na ausência de fatores de risco para tal (2). Neste relato será discutido o caso de uma paciente de 57 anos com diagnóstico inicial de abscesso hepático piogênico por K. pneumoniae, que evoluiu com disseminação hematogênica a despeito de terapia antimicrobiana guiada. Diagnóstico de ventriculite, meningite, espondilodiscite e mielite com abscesso intramedular foi realizado com ressonância magnética (RM) de sistema nervoso central (SNC) e neuroeixo. Testes fenotípicos e exames moleculares confirmaram cepa hipervirulenta. A despeito de grande inóculo bacteriano, paciente apresentou boa evolução com tratamento conservador de 8 semanas com carbapenêmico em monoterapia. ...
