Embasamento. A hipertensão arterial pulmonar (HAP) em pacientes (pcts) com esclerose sistêmica progressiva (ESP)/esclerodermia é a principal causa de morte. A identificação precoce da HAP permanece um desafio para a sobrevida e qualidade de vida dessa população. Objetivo. Avaliar a alteração funcional, se houver, de HAP assintomática ou não gravemente limitante em pcts com ESP com avaliação de variáveis de ecocardiograma de stress (SE) e teste de exercício cardiopulmonar (TECP) simultaneamente conduzidos em exercício com bicicleta ergométrica. Métodos. Estudo clínico, transversal, observacional, caso/controle de pcts com ESP vs saudáveis através de SE e TECP em cicloergômetro, avaliando principalmente a alteração da pressão sistólica artéria pulmonar (PSAP), reserva ventilatória e obtenção de padrão de exercício máximo, observada pelo limiar de lactato (LL) e captação de oxigênio (VO2) (pico). Resultados. Dezenove pcts com ESP (média 52,58±11,27 anos, sexo feminino 17 (89.5%), grupo caso, foram comparados em repouso/pico no SE e TECP. Igual número de voluntários sadios, os controles (média 32,10±6,67, sexo masculino 17 (89.4%) foi estudado. A duração do exercício não diferiu (caso) 9,2±2,93 vs (controle) 10,14±1,78 min, e as variáveis calculadas exibiram: taxa de trabalho (TT) (Watts) (% Pred) 57,16±17,39 vs 104,35±29,27; captação de oxigênio (VO2) (pico) 1,23±0,55 vs 2,45±0,7; VO2 (% pred) 80,21±18,22 vs 99,70±23,66; coeficiente respiratório (CR) (pico) 1,10±0,10 vs 1,17±0,09; razão equivalente respiratório pelo gás carbono (VE/VCO2) 31.90±3.65 vs 23.79±2.64; tempo aceleração fluxo pulmonar (TAC) 100,14±10,63 vs 108,86±6,38 (repouso) e 81.9±6,54 vs 73,39±5,18 (pico); velocidade regurgitação tricúspide (VRT) (m/s) 2.48±0.14 vs 2.08±0.12 (repouso) e 3,33±0.32 vs 2.34±0.09 (pico); permitindo o cáulculo da pressão sistólica artéria pulmonar (PSAP) de 33.32±6.49 vs 28.47±3. Conclusões. O TAC foi superior à VRT durante o SE-exercício para avaliação da PSAP em que a exequibilidade da obtenção da VRT e quantificação da pressão sistólica da artéria pulmonar é menor devido à hiperventilação e movimentação do tórax no cicloergômetro. Os pacientes com esclerodermia não interromperam o teste devido ao comprometimento pulmonar causado por esta doença. ...

Background. Pulmonary arterial hypertension (PAH) in patients (pts) with progressive systemic sclerosis (ESP)/scleroderma is the main cause of death. The early identification of PAH remains a challenge for the survival and quality of life of this population. Objective. To assess the functional change, if any, of asymptomatic or non-severely limiting PAH in pts with ESP with stress echocardiography (SE) and cardiopulmonary exercise testing (CPET) conducted simultaneously Methods. Clinical, cross-sectional, observational, case/control study of patients with ESP vs healthy patients using SE and CPET on a cycle ergometer to study the change in pulmonary artery systolic pressure (PASP), ventilatory reserve, and obtaining maximal exercise pattern, observed by the threshold lactate (LL) and oxygen uptake (VO2) (peak). Results. Nineteen patients with ESP (mean 52.58±11.27 years, female 17 (89.5%) had a rest/peak comparison in SE and CPET in equal group size in controls (mean 32.10±6.67, male 17 (89.4%). Exercise duration did not differ 9.2±2.93 vs 10.14±1.78 minutes, and the calculated variables showed: work rate (TT) (Watts) (% Pred) 57.16± 17.39 vs 104.35±29.27; oxygen uptake (VO2) (peak) 1.23±0.55 vs 2.45±0.7; VO2 (% pred) 80.21±18.22 vs 99.70±23.66; respiratory coefficient (CR) (peak) 1.10±0.10 vs 1.17±0.09; respiratory equivalent ratio for carbonic gas (VE/VCO2) 31.90±3.65 vs 23.79±2.64; pulmonary flow acceleration time (ACT) 100.14±10.63 vs 108.86±6.38 (rest), and 81.9±6.54 vs 73.39±5.18 (peak); tricuspid regurgitation velocity (TRV) (m/s) 2.48±0.14 vs 2.08±0.12 (rest) and 3.33±0.32 vs 2.34±0.09 (peak); allowing the calculation of pulmonary artery systolic pressure (PASP) of 33.32±6.49 vs 28.47±3.93 (rest) and 53.34±7.88 vs 43.33±5.49 (peak), p<0.05. There was no significant difference in the ventilatory reserve obtained by the formula (⩒E Max/MVV) maximal respiratory equivalent by maximal ventilatory ventilation 1.93±0.60 vs 1.76±0.36 (p=NS). Conclusions. ACT was superior to TRV during SE-exercise for PASP assessment in which the feasibility of obtaining TRV and quantifying pulmonary artery systolic pressure is lower due to hyperventilation and chest movement on the cycle ergometer. Patients with scleroderma did not discontinue the test due to the pulmonary impairment caused by this disease. ...
