dc.description.abstract | Introdução: A epilepsia do lobo temporal (ELT) é a mais frequente forma de epilepsia focal e está associada a elevados índices de transtornos cognitivos, comportamentais e/ou psicossociais. Foi demonstrado que não só o controle das crises, como também o tratamento das comorbidades psiquiátricas e o ambiente psicossocial são importantes para a qualidade de vida das pessoas com epilepsia. No entanto, muitas vezes indivíduos com sintomas depressivos são refratários ao tratamento farmacológico. A Estimulação Transcraniana por Corrente Contínua Domiciliar (ETCCd), pode ser uma interessante opção terapêutica não farmacológica para o tratamento de transtorno de humor em pessoas com ELT e por isso precisa ser melhor estudada. Objetivo: Avaliar o efeito do tratamento com ETCCd nos sintomas depressivos de pacientes com ELT. Métodos: Este ensaio clínico duplo-cego randomizou 26 adultos com ELT e sintomas depressivos em dois grupos simétricos: ETCCd ativa ou sham. Os participantes utilizaram a ETCCd por 20 minutos diários, 5 dias por semana durante 4 semanas, seguida de manutenção da aplicação da ETCC no laboratório de pesquisa 1 vez por semana por 3 semanas. A corrente aplicada foi de 2mA, bilateralmente sobre o córtex pré-frontal dorsolateral sendo o ânodo posicionado à esquerda e o cátodo à direita. Os participantes foram avaliados nos dias 1, 15, 30 e 60 do estudo, utilizando o Inventário de Depressão de Beck II (BDI). Uma avaliação de seguimento foi realizada cerca de 1 ano após o término do estudo. Resultados: Os grupos não diferiram em relação às características clínicas, socioeconômicas e psicométricas na avaliação inicial. Embora não tenha havido diferença estatisticamente significativa entre os grupos em relação aos efeitos adversos relatados, frequência das crises ou abandono do estudo, em média os participantes realizaram apenas 13 sessões eficazes de ETCC em casa em ambos os grupos. Houve muitas faltas nas avaliações devido à indisponibilidade dos participantes em retornar ao centro de pesquisa. Em média, entre o 1º e o 60º dia, o escore do BDI diminuiu 43,93% no grupo ativo e 44,67% no grupo Sham (ΔBDIfinal - inicial = -12,54 vs -12,20, p = 0,68). A melhora semelhante nos sintomas depressivos entre os dois grupos pode ser decorrente da interação entre os participantes e o grupo de pesquisa associada ao efeito placebo da intervenção, e não apenas à intervenção ETCCd em si. Estes resultados apresentam a problemática de aderência a esse tipo de tratamento e incentivam o estudo de melhores intervenções não-farmacológicas para o tratamento desses pacientes. | pt_BR |
dc.description.abstract | Introduction: Temporal Lobe Epilepsy (TLE) is the most frequent form of focal epilepsy and is associated with high rates of cognitive, behavioral and psychosocial disorders. It has been shown that not only the control of crises, but also the treatment of psychiatric comorbidities and the psychosocial environment are important for the quality of life of people with epilepsy. However, individuals with depressive disorder do not always respond to treatment with antidepressant medications. Home-Based Transcranial Direct Current Stimulation (tDCS-hb), can be an interesting non-pharmacological therapeutic option for the treatment of mood disorder in people with TLE and therefore needs to be better studied. Objective: To evaluate the effect of treatment with ETCCd on depressive symptoms in patients with TLE. Methods: This double-blind clinical trial randomized 26 adults with TLE and depressive symptoms into two symmetrical groups: active or sham tDCS-hb. Participants used tDCS-hb for 20 minutes daily, 5 days a week for 4 weeks, followed by maintaining the application of tDCS in the research laboratory once a week for 3 weeks. The applied current was 2mA, bilaterally over the dorsolateral prefrontal cortex with the anode positioned on the left and the cathode on the right. Participants were assessed on days 1, 15, 30 and 60 of the study, using the Beck Depression Inventory II (BDI). A follow-up evaluation was carried out approximately 1 year after the end of the study. Results: The groups did not differ in relation to clinical, socioeconomic and psychometric characteristics in the initial assessment. Although there was no statistically significant difference between the groups in relation to the reported adverse effects, frequency of seizures or dropping out of the study, on average the participants had only 13 effective sessions of tDCS at home in both groups. There were many absences in the evaluations due to the unavailability of the participants to return to the research center. On average, between the 1st and the 60th day, the BDI score decreased by 43.93% in the active group and 44.67% in the Sham group (ΔBDIfinal - initial = -12.54 vs -12.20, p = 0, 68). The similar improvement in depressive symptoms between the two groups was attributed to the interaction between the participants and the research group associated with placebo effect, rather than the tDCS-hb intervention itself. These results present the problem of adherence to this type of treatment and encourage the study of better non-pharmacological interventions for the treatment of these patients. | en |
