Cuidado à criança vivendo com HIV e família : perspectiva da advocacia em saúde

Abstract

Este estudo está inserido na linha de pesquisa Cuidado de enfermagem na saúde da mulher, criança, adolescente e família, vinculado ao Grupo Cuidado à Saúde nas Etapas da Vida (CEVIDA) do Programa de Pós-Graduação em Enfermagem da Universidade Federal do Rio Grande do Sul (UFRGS). Teve-se o objetivo de conhecer e compreender as ações de advocacia em saúde desenvolvidas pelos profissionais no cuidado à saúde da criança vivendo com HIV e familiares/cuidadores. Foi realizada uma pesquisa com abordagem qualitativa em dois serviços ambulatoriais de atendimento especializado em IST/AIDS, com atendimento à criança vivendo com HIV, na cidade de Porto Alegre, RS/BR. Participaram do estudo sete profissionais de saúde que compõem as equipes desses dois serviços. A coleta de informações ocorreu em duas etapas: a primeira englobou entrevista individual, e a segunda constituiu-se de Dinâmicas de Criatividade e Sensibilidade (DCS) com os participantes de cada serviço. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa da UFRGS (instituição proponente) CAAE: 05216018.6.3001.5530, e pelos Comitês de Ética em Pesquisa das Instituições coparticipantes; Comitê de Ética em Pesquisa do Grupo Hospitalar Conceição (CEP-GHC) e Comitê de Ética em Pesquisa da Prefeitura Municipal de Porto Alegre (CEP-PMPA). Os participantes do estudo, após o aceite, assinaram o Termo de Consentimento Livre e Esclarecido, conforme Resolução n° 466/12. A partir da interpretação das informações emergiram as dimensões sobre a defesa da criança vivendo com HIV em três níveis de significados: Dimensão Equipe de Saúde; Dimensão Cuidado na Perspectiva da Vulnerabilidade; Dimensão Advocacia em Saúde. As ações de advocacia em saúde desveladas foram: trabalho em equipe; rede de saúde; cuidado em saúde; reconhecimento das situações de vulnerabilidade; significado da defesa da saúde; articulação do direito. Já as barreiras foram: recursos humanos e incipiente articulação dos direitos. A busca do direito à saúde da criança com HIV pela equipe de saúde está pautada na inter-relação da tríade interdisciplinaridade, intersetorialidade e integralidade, ao considerar a complexidade relacionada ao HIV, ultrapassando marcos do setor saúde e tendo maior capacidade de resolutividade. Como inovação e contribuição para a enfermagem e equipe de saúde este estudo propõe a incorporação do conhecimento dos direitos de proteção já existentes: Constituição Federal, Princípios do SUS, Advocacia em saúde, Estatuto da Criança e do Adolescente, Convenção sobre os Direitos das Crianças, Dimensões Individual, Social e Programática da Vulnerabilidade, associada à tríade interdisciplinaridade, a intersetorialidade e a integralidade. As ações propostas por esta pesquisa configuram-se em estratégia de cuidado à saúde para advogar em prol do direito da criança vivendo com HIV e família, oferecendo aos profissionais de saúde suporte e recursos para o desenvolvimento de ações de cuidado.

This study falls within the research line Nursing Care in Woman, Child, Adolescent and Family`s Health in connection with the Group of Healthcare upon Life Stages (CEVIDA) of the Post-Graduation Program in Nursing of the Federal University of Rio Grande do Sul (UFRGS). The objective was learning and understanding advocacy actions in health as developed by professionals within health care of children with HIV, their families and caretakers. A research with qualitative approach was carried out in two outpatient services of specialized care in IST/AIDS attending children with HIV in the city of Porto Alegre, RS/BR. The study had the participation of seven health professionals of the staffs from these two services. The collection of information was done in two stages: the first one made individual interview while the second one applied Creativity and Sensibility Dynamics (DCS) with the participation of each service staff. The research was approved by the Committee of Ethics in Research of UFRGS (proposing institution) CAAE: 05216018.6.3001.5530, and by the Committees of Ethics in Research of the co-participating Institutions; Committee of Ethics in Research of Conceição Hospital Group (CEP-GHC) and Committee of Ethics in Research of the Municipal Administration of the City of Porto Alegre (CEP-PMPA). After the study has been accepted, the participants signed the Free and Informed Consent Form as per Resolution no. 466/12. After the information has been interpreted, the following dimensions on the defense of children with HIV have emerged at three meaning levels: Health Staff Dimension; Care Dimension from the Vulnerability Perspective; Health Advocacy Dimension. Health advocacy actions have been identified as contributors for the qualification and materialization of the integral health care of children with HIV and their family as well as the difficulties met by the staffs that have been configured as barriers for the health advocacy. The unveiled health advocacy actions were: staff work; health network; health care; recognition of vulnerability situations; meaning of health defense; law articulation. As to the barriers, evidences showed: human resources and incipient articulation of rights. The search of the right to health for children with HIV by the health staff is based on the interrelation of the triad interdisciplinarity, intersectorality and integrality when considering the HIV-related complexity that overpasses the framework of the health sector with higher problem-solving capacity. As innovation and contribution for nursing and the health staff, this study proposes the incorporation of knowledge on the protection rights already existing: Federal Constitution, SUS Principles, Health Advocacy, Child and Adolescent Statute, Convention on Children`s Rights, Individual, Social and Programmatic Dimensions of Vulnerability in connection with the triad interdisciplinarity, intersectorality and integrality. The actions proposed by this research configure a health care strategy to advocate in favor of the rights of children with HIV and their family by offering health professionals the support and resources needed to develop care actions.
