dc.description.abstractAs infecções urinárias possuem como principal agente causador a bactéria Escherichia coli. Esta, possui um potente arsenal de mecanismos de virulência capazes de modular a sua entrada e fixação no urotélio. Para o tratamento deste tipo de infecção, a antibioticoterapia é amplamente utilizada. No entanto, devido ao crescente número de cepas bacterianas resistentes causadas pelo mal-uso de antibióticos, novas intervenções clínicas, vem sendo estudadas a fim de auxiliar o tratamento destas infecções, como é o caso das Proantocianidinas do Vaccinium macrocapon (Cranberry). Desta forma, o objetivo deste estudo é realizar um levantamento bibliográfico com as evidências clínicas em relação ao cranberry, bem como sobre a sua relação e mecanismos de ação envolvidos nas infecções urinárias. O cranberry é uma fruta nativa do leste da América do Norte, mas está também presente no Norte da Europa e Norte Asiático. Esta fruta encontra-se para o consumo humano sob a forma de baga, suco, xarope, cápsulas ou comprimidos, sendo estes dois últimos normalmente com concentrações estandardizadas de compostos bioativos. Dentre a sua composição, podemos citar as proantocianidinas (PACs), principal fitoconstituinte, sobretudo do tipo A que representa aproximadamente 94,5% das PACs totais presentes no cranberry. Este fitoconstituinte, é responsável por modular as infecções urinárias através da inibição da adesão bacteriana. As concentrações entre 36 e 72mg de PACs do tipo A, administradas em estudos clínicos, foram as que obtiveram melhores resultados, possuindo efeito ao tratamento profilático e redução de piúria. As PACs tipo A possuem mecanismos que articulam o processo anti-aderência a nível fímbrial, principalmente em Escherichia coli. Além disso, o nível de piúria é regulado pelas PACs tipo A, através das citocinas, enzimas, proteínas e outras vias de expressão ligadas ao desenvolvimento inflamatório. No entanto, existem algumas controvérsias em relação a dosagem padronizada de consumo de PACs tipo A, em alguns grupos populacionais, como por exemplo, em idosos e pacientes com alguma patologia crônica. Sendo assim, torna-se necessário a realização de novos estudos a respeito sobre as dosagens para o consumo humano em diferentes grupos populacionais, acrescendo valor teórico e melhor elucidação para a prática clínica. O cranberry mostra-se como um importante e promissor adjuvante ao tratamento e profilaxia das infecções urinárias. No entanto, torna-se importante estudar o possível efeito das PACs tipo A em diferentes espécies de bactérias causadoras de infecções urinárias, a fim de expandir o conhecimento além de sua ação sobre E. coli.pt_BR
dc.description.abstractAs urinary infections have Escherichia coli as the main causative agent. This bacterium, like others, has a potent arsenal of virulence mechanisms capable of modulating its entry and correction in the uroepithelium. For the treatment of this type of infection, it is widely used as antibiotic therapy. However, due to the growing number of resistant bacterial strains caused by the misuse of antibiotics, new clinical techniques have been studied in order to help the treatment originated, as is the case of Vaccinium macrocapon (Cranberry). Thus, the objective of this study is to carry out a bibliographic survey with the clinical evidence in relation to the cranberry, as well as on its relationship and actions performed in urinary editions. Cranberry is a fruit native to eastern North America, but is also present in northern Europe and northern Asia. This fruit is available for human consumption in the form of berry, juice, syrup, capsules or tablets, the latter two of which are normally standardized with bioactive compounds. Among its composition, we can mention as proanthocyanidins (PACs), the main phytochemical constituent, especially as type A PACs, representing about 94.5% of the total PACs present in cranberry. This phytoconstituent is responsible for modulating urinary changes by its mechanism of action, inhibiting bacterial adhesion. The restrictions between 36 and 72mg of type A PACs, administered in clinical studies, were those that obtained the best results, having the effect on prophylactic treatment and reduction of pyuria. As type A PACs have mechanisms that articulate the anti-adherence process at the fimbrial level, especially in Escherichia coli. In addition, the level of pyuria is regulated by PACs type A, through cytokines, enzymes, proteins and other expression pathways linked to inflammatory development. However, there are some controversies regarding the standardized dosage of consumption of type A PACs in some population groups, for example, in the elderly and patients with some chronic pathology. Thus, it is necessary to carry out a theoretical complement through new studies on dosages for human consumption in different population groups, adding theoretical value and better clarification for a clinical practice. In addition, look for the possible effect of type A PACs on different bacteria that cause urinary infections, since Cranberry is based mainly on its mechanism of action in Escherichia coli. In addition, cranberry is shown as an important and promising adjuvant to the treatment and prophylaxis of urinary languages.en
