dc.description.abstractIntrodução: Apesar dos avanços atuais no tratamento e monitorização de pacientes com insuficiência cardíaca (IC), os resultados após a hospitalização ainda estão abaixo das expectativas. É difícil manter os benefícios de estudos no cenário real, principalmente a médio e longo prazo. Por esse motivo, são necessárias estratégias de transição de cuidados mais pragmáticas, principalmente para que seus resultados se prolonguem por mais tempo. Este estudo foi desenvolvido para preencher esta lacuna no cenário de pacientes com IC. Objetivo: Avaliar a efetividade da promoção do conhecimento e do autocuidado em IC, utilizando uma estratégia automatizada de telemonitoramento baseada no envio de SMS (Short Message Service). Métodos: Subanálise de um ensaio clínico randomizado multicêntrico. O Grupo Experimental (GE) e o Grupo Controle (GC) receberam instruções para a alta hospitalar conforme padrões usuais das instituições, mais um guia de orientações sobre IC e um agendamento de visita de acompanhamento de 30 dias. Apenas o GE recebeu um guia que explicava sobre as mensagens de texto diárias, uma balança e um aparelho de pressão. Os desfechos desta subanálise foram o Conhecimento da IC e Autocuidado, extraídos do banco de dados do estudo principal. Para avaliar o conhecimento foi aplicado um questionário composto por 14 perguntas, que considera que o paciente tem conhecimento adequado com 70% das respostas corretas. As habilidades para o autocuidado foram mensuradas usando a Escala Europeia de Comportamento de Autocuidado para Insuficiência Cardíaca (EHFScBs) versão brasileira, composta por 12 itens em que a pontuação máxima a ser atingida é de 60 pontos; quanto menor a pontuação, melhor o autocuidado. Resultados: Em uma amostra de 117 pacientes, a idade média no GE foi de 60 ± 12 e 59 ± 12 anos no GC; em ambos os grupos, os pacientes foram predominantemente do sexo masculino (39 (66%) no GE e 34 (58%) no GC; 47% tinham ensino fundamental incompleto; O escore basal do questionário de conhecimento de IC foi 71% ± 21 no GE e 69 % ± 17 para o GC e, para o questionário de autocuidado, foram 27 ± 9 e 26 ± 8 pontos, respectivamente. O escore final de conhecimento após 30 dias de seguimento não diferiu significativamente entre o GE e o GC. O GE aumentou 11,1 pontos e o GC 11,6 pontos no escore (83% ± 16 vs 81% ± 16; p = 0,908). O escore final de autocuidado após 30 dias de seguimento diferiu significativamente entre o GE e o GC. O GE diminuiu 6,1 pontos e o GC diminuiu 3,5 pontos no escore (20 ± 7 vs 22 ± 8; p = 0,014). O conhecimento aumentou significativamente em todos os pacientes em 30 dias, independentemente do grupo de alocação, 70% ± 19 para 82% ± 16; p <0,001. Conclusão: Ao final de 30 dias, o conhecimento não aumentou significativamente entre os grupos. No entanto, houve um aumento significativo no autocuidado em pacientes no GE. Como forma de promover o autocuidado da IC, a intervenção por SMS combinando com consultas, aparelhos para verificar parâmetros hemodinâmicos e um guia de orientações sobre IC pode ser uma estratégia adequada de transição de cuidado após a alta de pacientes internados por IC agudamente descompensada. Mais estudos cuidadosamente projetados com amostras robustas são necessários para confirmar esses achados.pt_BR
dc.description.abstractIntroduction: Despite the current advances in the treatment and monitoring of patients with heart failure (HF), the results after hospitalization are still below expectations. It is difficult to maintain the benefits of studies in the real world, especially in the medium and long term. For this reason, more pragmatic care transition strategies are needed, especially so that their results can continue for longer. This study was developed to fill this gap in the scenario of patients with HF. Objective: To evaluate the effectiveness of promoting HF knowledge and self-care, using a telemonitoring strategy based on the sending of SMS (Short Message Service). Methods: Subanalysis of a randomized multicenter clinical trial. The Experimental Group (EG) and the Control Group (CG) received instructions for hospital discharge according to the usual standards of the institutions, plus a guidance guide on HF and a 30-day follow-up visit schedule. Only EG received a guide that explained about daily text messages, a scale and a pressure device. The outcomes of this sub-analysis were HF Knowledge and self-care, extracted from the main study database. To assess knowledge, a questionnaire consisting of 14 questions was applied, which considers that the patient has adequate knowledge with 70% of the correct answers. The skills for self-care were measured using the European Self-Care Behavior Scale for Heart Failure (EHFScBs) Brazilian version, composed of 12 items in which the maximum score to be achieved is 60 points; the lower the score, the better the self-care. Results: In a sample of 117 patients, the mean age in the EG was 60 ± 12 and 59 ± 12 years in the CG; In both groups, the patients were predominantly male (39 (66%) in the EG and 34 (58%) in the CG; 47% had incomplete primary education; the baseline score of the CI knowledge questionnaire was 71% ± 21 in the EG and 69% ± 17 for the CG and, for the self-care questionnaire, there were 27 ± 9 and 26 ± 8 points respectively. The final knowledge score after 30 days of follow-up did not differ significantly between the EG and the CG. EG increased by 11.1 points and the CG by 11.6 points in the score (83% ± 16 vs 81% ± 16; p = 0.908). The final self-care score after 30 days of follow-up differed significantly between the EG and the CG. The EG decreased 6.1 points and the CG decreased 3.5 points in the score (20 ± 7 vs 22 ± 8; p = 0.014). Knowledge increased significantly in all patients within 30 days, regardless of the allocation group, from 70% ± 19 to 82% ± 16; p <0.001 Conclusion: At the end of 30 days, knowledge did not increase significantly between groups. However, there was a significant increase in self-care in patients in the EG. As a way of promoting self-care for HF, SMS intervention combining consultations, devices to check hemodynamic parameters and a guide for HF guidance can be an appropriate strategy for the transition of care after discharge of hospitalized patients due to acute decompensated heart failure. More carefully designed studies with robust samples are needed to confirm these findings.en
