Escape posterior tardio na deglutição : comparação de adultos e idosos

Abstract

Introdução: O envelhecimento tem repercussões na fisiologia da deglutição. Há um declínio funcional do ato de deglutir que repercute na fase oral, faríngea e esofágica com o avançar da idade. Em um estudo anterior caracterizamos um evento não descrito na literatura, o escape posterior tardio na deglutição (EPT). A hipótese da equipe é que o evento em estudo seja uma característica da presbifagia, na qual se refere ao envelhecimento natural que aflige a funcionalidade do sujeito. Objetivo: A pesquisa objetiva comparar a deglutição de adultos e idosos a fim de relacionar o evento em estudo ao envelhecimento e excluir o viés de possível evento normal da deglutição. Método: Trata-se de um estudo transversal quantitativo e qualitativo contemporâneo com enfoque diagnóstico que será realizado no Hospital Público de referência de Porto Alegre, Brasil. A coleta de dados ocorreu através da análise dos exames realizados. Resultado: O evento estudado, EPT, ocorreu em 75% (15) do total de 20 exames realizados com idosos e em 25% (5) com adultos, sendo que esses que apresentaram estão acima de 55 anos. Alta relação de queixa relatada pelo individuo com a alteração no exame objetivo. Conclusão: As queixas dos pacientes e a ocorrência do EPT ocorreram predominantemente na consistência sólida e líquida, nos indivíduos idosos ou adultos com idade próxima a 60 anos. O EPT faz parte de um declínio funcional da deglutição.

Introduction: As we age, the human body undergoes several structural and functional modifications. Aging has repercussions on the physiology of swallowing. There is a functional decline in the act of swallowing that affects the oral, pharyngeal and esophageal phase with advancing age. In a previous study we characterized an event not described in the literature, the late posterior escape in swallowing, in which we characterized as a food residue that descends through the tongue base after physiologically finalized swallowing. The team hypothesis is that the event under study is a characteristic of presbyphagia, in which it refers to the natural aging that afflicts the subject's functionality. Objective: The research aims to compare the swallowing of adults and the elderly in order to relate the event under study to aging and exclude the bias of a possible normal event of swallowing. Methods: This is a contemporary quantitative and qualitative cross-sectional study with a diagnostic focus that will be performed at the Hospital Clínicas de Porto Alegre (HCPA). Data collection will occur through the analysis of the exams performed. The images will be studied individually by the researcher, aiming to analyze the effectiveness and safety of feeding and to identify and select images that detect late posterior escape in swallowing. Result: The studied event, EPT, occurred in 75% (15) of the total of 20 exams performed with the elderly and in 25% (5) with adults, and those who presented are over 55 years old. High complaint ratio reported by the individual with the change in the objective examination. Conclusion: Patients' complaints and the occurrence of EPT occurred predominantly in solid and liquid consistency, in elderly individuals or adults aged close to 60 years. EPT is part of a functional decline in swallowing.
