Os tecidos endodôntico e periodontal estão intimamente ligados. Quando ambas patologias estão presentes em um mesmo dente, com relação de coexistência e interdependência, temos a lesão endo-periodontal, a qual representa um processo infecto-inflamatório único. O diagnóstico dessa patologia é um desafio para os cirurgiões-dentistas, uma vez que as doenças periodontal e endodôntica não somente podem ser uma consequência isolada de uma infecção presente em um desses tecidos, mas também podem mimetizar características uma da outra. O presente trabalho relatou o tratamento de um caso de lesão endo-periodontal. Em 2013, a paciente apresentava doença periodontal generalizada e foi diagnosticada com uma lesão endo-periodontal no dente 14. Assim, tratamentos combinados foram instituídos: a paciente recebeu tratamento endodôntico no dente 14 na Especialização de Endodontia da UFRGS e depois foi tratada periodontalmente na Especialização de Periodontia da mesma instituição. Dois anos depois (em 2015), após tratamento convencional para a doença endo-periodontal, o caso apresentou insucesso. Dessa maneira, elaboramos duas possibilidades de diagnóstico para o insucesso: lesão endo-periodontal no dente 14 com origem endodôntica primária e envolvimento periodontal secundário; ou lesão endo-periodontal verdadeira combinada. Este diagnóstico levou à decisão de uma abordagem endodôntica (retratamento) + cirurgia periodontal exploratória. Após abordagem endodônticocirúrgica, e acompanhamento do caso por 6 meses, obtivemos sucesso e fomos capazes de entender que a causa do insucesso (defeito ósseo no dente 14) foi o biofilme periapical presente no pré-molar. Assim, o presente trabalho objetivou enriquecer a literatura no que concerne às dificuldades e desafios em relação ao diagnóstico e tratamento das lesões endo-periodontais. Além disso, este relato de caso vem lembrar que o profissional deve estar atento sempre ao tratamento de eleição baseado em evidências científicas, mas deve estar preparado também para lidar com casos específicos e que apresentam insucesso da terapia padrão, muitas vezes utilizando tratamento cirúrgico. ...
Abstract
The dental pulp and periodontal tissues are intimately related. When endodontic and periodontal diseases affect a same tooth, having an interdependence relationship, it results in a combined endodontic-periodontal lesion. Such a lesion presents an unique inflammatory process. Often, the diagnosis of endodontic-periodontal diseases represents a challenge for the clinicians, due to not only the endodontic and periodontal lesions might be a consequence of an infection on this tissues, but also, the diseases can mix characteristics from one to another. The current clinical case reported the treatment of an endodontic-periodontal disease. In 2013, the patient had generalized periodontal disease, and an endodontic-periodontal lesion on tooth 14. Then, combined treatment for both diseases were performed: endodontic treatment for the tooth 14 in the Endodontic Specialty Clinic, and after that, periodontal treatment in the Periodontal Specialty Clinic at UFRGS. After two years (in 2015), following the conventional treatment for the endodontic-periodontal disease, the clinical case failed. Thus, two possibilities of diagnosis for the case failure were elaborated: endodontic-periodontal lesion on the tooth 14 with primary endodontic origin and secondary periodontal involvement; or true combined endodonticperiodontal lesion. These possibilities of diagnosis lead to a decision-making that combines root canal retreatment and exploratory periodontal surgery. After the endodontic approaching (root canal retreatment) + exploratory surgery, and follow up during 6 months, we obtained success for this clinical case and we were able to understand that the etiology for the failure (bone defect on tooth 14) was the biofilm in the periapical region. As a conclusion, the current case report aimed to add valuable information to the literature in regard the difficulties and challenge faced by the clinicians during the diagnosis and treatment of the endodontic-periodontal lesions. In addition, this article highlights not only the necessity to do an evidencebased treatment, but also the possibility of coping with unique cases which had a previous failed approaching, oftentimes, using a surgical intervention. ...
