Introdução: Estudos com adolescentes tem demonstrado baixa prevalência de síndrome metabólica (SM) utilizando os atuais critérios para diagnóstico. Para crianças ainda não existe classificação para esta condição, sendo utilizados pontos de corte adaptados para a população adulta ou apenas dados do histórico familiar. Além disso, os atuais critérios não apresentam concordância no diagnóstico, não havendo um consenso do melhor a ser utilização para a população infanto-juvenil. Alguns estudos internacionais tem utilizado um novo critério, baseado na avaliação do risco metabólico (RM), por peio de um escore contínuo (continuous metabolic risk score; cMetS). Porém, na população infanto-juvenil brasileira, este escore ainda foi pouco explorado. Objetivo: Avaliar a concordância entre os principais critérios de diagnóstico de SM e a validade do cMetS em adolescentes de 10 a 17 anos de idade e testar a aplicação deste escore em crianças com menos de 10 anos. Método: Estudo transversal composto por uma amostra de 2400 escolares, sendo 1739 adolescentes, de ambos os sexos, do município de Santa Cruz do Sul-RS. Foram utilizados três diferentes critérios para o diagnóstico da SM em adolescentes: International Diabetes Federation (IDF), Ferranti e Cook. A concordância entre os critérios foi avaliada por meio do teste de Kappa. O cMetS foi calculado utilizando a soma do escore Z de seis fatores de risco às doenças cardiovasculares (DCV): circunferência da cintura (CC), pressão arterial sistólica (PAS), níveis séricos de triglicerídeos (TG), colesterol de alta densidade (HDL-c; high density lipoprotein) e glicose, e aptidão cardiorrespiratória (APCR). Os valores de HDL-c e APCR foram multiplicados por -1, por indicarem uma relação inversa com os fatores de risco às DCV. Foram criados pontos de corte para o cMetS, considerando como referência os três critérios para SM, por meio da curva ROC (receiver operator characteristic). Para crianças, foi realizado o agrupamento dos fatores de risco às DCV. Foram considerados como RM os valores de CC, PAS, TG e glicose no último quartil e de HDL-c e APCR no primeiro quartil. Os valores médios do cMetS foram comparados de acordo com o número de fatores de risco agregados. A análise de componentes principais também foi utilizada para avaliar a agregação entre os fatores de risco nas crianças. Resultados: Foi encontrada baixa prevalência de SM nos adolescentes (1,9% para Cook, 5,0% para Ferranti e 2,1% para IDF). Foi demonstrada concordância regular (Ferranti – IDF) e moderada (Cook – Ferranti; Cook – IDF) entre os critérios. A alteração na PAS foi a condição mais frequente em todos os critérios e as menos frequentes foram nos níveis séricos de glicose (Cook e Ferranti) e de HDL-c (IDF). O melhor ponto de corte para o cMetS foi estabelecido em 3,40 para meninos (sensibilidade: 100,0%; especificidade: 92,9%; AUC: 0,978) e 3,61 para meninas (sensibilidade: 100,0%; especificidade: 93,1%; AUC: 0,991). Para estes pontos de corte, foi encontrado RM em 8,9% dos adolescentes (9,4% para o sexo masculino e 8,5% para o sexo feminino). Foi obtida relação linear entre os valores médios do cMetS com o número de componentes da SM (média de cMetS de -1,09 para nenhum componente presente e 6,66 para 3 componentes ou mais). Em crianças, o agrupamento dos fatores de risco foi evidenciado em 10% da amostra. O cMetS demonstrou relação linear com o aumento do número de fatores de risco às DCV (nenhum fator de risco: -3,29 para ambos os sexos; 5 fatores de risco: 7,45 para o sexo masculino e 7,25 para o sexo feminino). Conclusões: A baixa prevalência de SM e a baixa concordância entre os critérios existentes sugere a necessidade de elaboração de novos critérios para diagnóstico de SM na população infanto-juvenil. A utilização do cMetS parece ser válida para crianças e adolescentes e consegue detectar maior proporção de escolares com RM, em comparação aos atuais critérios para diagnóstico de SM. ...

Introduction: Studies with adolescents have shown a low prevalence of metabolic syndrome (MS) with the current criteria for diagnosis. For children, there is no classification for this condition, using adapted cutoff points for adults or using family history data. In addition, the current criteria do not present concordance in the diagnosis, and there is no consensus of the best criterion to be use for child and adolescent population. Thus, international studies have used a new approach, based on the continuous metabolic risk score (cMetS). However, in the Brazilian infant and juvenile population, this score was little explored. Objective: To evaluate the agreement between the main methods of diagnosis of MS and the validity of the cMetS in adolescents aged 10 to 17 years and to test the application of this score in children under 10 years. Method: a cross-sectional study comprised a sample of 2,400 schoolchildren, of these 1,739 are adolescents, of both sexes, from the city of Santa Cruz do Sul-RS. Three different criteria were used to diagnose MS in adolescents: International Diabetes Federation (IDF), Ferranti and Cook. The agreement between the criteria was evaluated through the Kappa test. cMetS was calculated using the sum of the Z score of six cardiovascular disease (CVD) risk factors: waist circumference (WC), systolic blood pressure (SBP), triglycerides (TG), high density lipoprotein cholesterol (HDL-c), glucose and cardiorespiratory fitness (CRF). HDL-c and CRF values were multiplied by -1, as they indicated an inverse relationship with the CVD risk factors. Cutoff points were created for cMetS, considering the three criteria for MS, using the receiver operator characteristic (ROC) curve. For children, the grouping of risk factors for CVD was evaluated. The values of WC, SBP, TG and glucose in the last quartile and of HDL-c and CRF in the first quartile were considered as metabolic risk. The mean values of cMetS were compared according to the number of clustered risk factors. Principal component analysis was also used to assess aggregation among risk factors in children. Results: There was a low prevalence of MS in adolescents (1.9% for Cook, 5.0% for Ferranti and 2.1% for IDF). Regular (Ferranti - IDF) and moderate (Cook - Ferranti; Cook - IDF) agreement was demonstrated among the criteria. High blood pressure was the most frequent condition in all criteria and the least frequent were high levels of glucose (Cook and Ferranti) and low levels of HDL-c (IDF). The best cutoff point for cMetS was set at 3.40 for boys (sensitivity: 100.0%, specificity: 92.9%, AUC: 0.978) and 3.61 for girls (sensitivity: 100.0%; specificity: 93.1%, AUC: 0.991). For these cutoff points, metabolic risk was found in 8.9% of adolescents (9.4% for boys and 8.5% for girls). A linear relationship was found between the mean values of cMetS and the number of components of MS (cMetS mean of -1.09 for no component present and 6.66 for three components or more). In children, the cluster of risk factors was evidenced in 10% of the sample. cMetS demonstrated a linear relationship with the increase in the number of risk factors for CVD (no risk factor: -3.29 for boys and girls, 5 risk factors: 7.45 for boys and 7.25 for girls), also valid for children. Conclusions: The low prevalence of MS and the low agreement between the existing criteria suggests the elaboration of new criteria for the diagnosis of MS for schoolchildren. The use of cMetS is valid for children and adolescents and is able to detect a greater proportion of students with metabolic risk, compared to the current criteria for diagnosis of MS. ...
