Técnica de avanço da tuberosidade tibial (TTA) para tratamento de ruptura de ligamento cruzado cranial em cães

Abstract

A ruptura de ligamento cruzado cranial (RLCCr) é uma das lesões ortopédicas mais frequentes em cães, e a maior causa de instabilidade e doença degenerativa articular da articulação fêmoro-tíbio-patelar. O diagnóstico é realizado através da anamnese juntamente com os achados do exame físico e ortopédico. Diversas técnicas já foram descritas visando o tratamento do RLCCr, porém a falta de estabilidade pós-operatória a longo prazo, juntamente com a progressão da osteoartrite, estimulou a busca por novas técnicas. A técnica descrita neste estudo, denominada de avanço da tuberosidade tibial (TTA), aparenta diminuir a pressão intra-articular do joelho, através da alteração da biomecânica desta articulação, o que resulta na diminuição da progressão da degeneração articular inerente desta lesão ligamentar. Este estudo objetiva abordar, através de revisão bibliográfica, os principais aspectos relacionados ao pré, trans e pós-operatório do uso do avanço da tuberosidade tibial (TTA) como tratamento cirúrgico para RLCCr em cães. Assim como relatar as consequências, as vantagens e desvantagens desta técnica de osteotomia corretiva e os avanços científicos e tecnológicos da área.

The rupture of the cranial cruciate ligament (RLCCr) is one of the most frequent injuries of hindlimbs lameness in dogs. It is the main cause of instability and degenerative joint disease of the femoral-tibial-patellar joint. The diagnosis is made by medical history along with the findings of physical and orthopedic examination. Numerous techniques have been described in order to treat the RLCCr, but the lack of long term post operative stability, along with the progression of osteoarthritis, stimulated the search for new techniques. The technique described in this study, called tibial tuberosity advancement (TTA) appears to decrease the knee intraarticular pressure, by changing the biomechanics of the knee joint, which results in decreasing the progression of joint degeneration inherent in this ligament injury. This study aims to report, through literature review, the main aspects related to the pre, trans and postoperative use of the tibial tuberosity advancement (TTA) as surgical treatment for rupture of the cranial cruciate ligament in dogs. As well as report the consequences, the advantages and disadvantages of this corrective osteotomy technique and scientific and technological advances in the area.
