Contexto: As Doenças Raras no Brasil são um tema recente junto aos órgãos regulató- rios governamentais. A Politica Nacional respectiva foi contemplada apenas em 2014 com a Portaria nº 199 do Ministério da Saúde. Objetivo: Descrever as características das internações na rede pública brasileira por Mielofibrose Aguda, uma doença rara, entre os anos de 2008 e 2012. Procedimentos metodológicos: Estudo epidemiológico de base populacional, observacional e transversal, com base nos dados do Sistema de Informações Hospitalares do SUS (SIH/SUS). Foram selecionadas internações relativas às competências janeiro de 2008 a dezembro de 2012, disponíveis publicamente no sit do DATASUS, cujo diagnóstico principal foi CID-10 C94.5. Cálculo de indicadores por sexo, faixas etárias, utilização de UTI e gastos por internação. Resultados: Foram ana- lisadas 501 AIHs (Autorizações de Internação Hospitalar) tipo 1 devidas a Mielofibrose Aguda (0,53/milhão de habitantes/ano) cuja letalidade foi de 11,6% (58 óbitos) e coefi- ciente de mortalidade hospitalar de 0,06/milhão de habitantes/ano. O total de interna- ções distribuiu-se em 72,5% para homens e 27,5% para mulheres. A faixa etária de 65 a 69 anos foi a que mais utilizou dias de internação, coincidindo com a mediana de mor- te devido à doença. O tempo médio de permanência foi de 8,7 dias, com um custo total de R$ 2,9 milhões no período analisado e R$ 5,9 mil por internação. Entre as AIHs para pacientes do sexo masculino, os óbitos representaram 10,7%, enquanto para o sexo feminino 13,8%. As mulheres registraram em média 12,5 dias de internação, ou seja, 5,3 dias a mais do que os homens (7,2 dias). O gasto médio por internação foi de R$ 5.856,17 e os gastos médios das pacientes do sexo feminino (R$ 9.125,37) foram maio- res do que os do sexo masculino (R$ 4.613,34). A região sul do Brasil registrou 87,2% das 501 AIHs analisadas. Dentre as 86 AIH oriundas do Rio Grande do Sul, 60 tiveram origem em Santa Cruz do Sul. Conclusão: Ainda que o total de internações no Brasil tenha correspondido a apenas cerca de 100 por ano, a Mielofibrose Aguda, por atingir população idosa, apresenta um potencial de expansão devido à transição demográfica, a melhores métodos diagnósticos, e a expectativa de crescimento desse segmento po- pulacional. ...
