Artefatos em tomografia computadorizada : revisão de literatura e relato de caso

Abstract

Sabe-se que os aparelhos de tomografia computadorizada por feixe em leque (TCFL) e tomografia computadorizada por feixe cônico (TCFC) apresentam características diferentes, entretanto, ambas possuem indicações e limitações. Em odontologia, a TCFC é mais utilizada, devido à menor dose de radiação e ao menor custo do exame. Entretanto, ela apresenta desvantagens, tais como área de alcance dos raios X (campo de visão) limitada em alguns aparelhos e a perda da qualidade da reconstrução tridimensional. Em ambos os tipos de tomografia computadorizada ocorre a formação de artefatos na imagem. Em tomografia computadoriza (TC), a palavra artefato é usada para definir a discrepância entre os verdadeiros números dos objetos e o coeficiente de atenuação nas imagens reconstruídas. Os artefatos podem interferir negativamente no diagnóstico, dificultando a visualização da região de interesse ou simulando alterações. As origens dos artefatos são diversas, entre elas, pode-se citar fatores físicos do aparelho, do paciente e das reconstruções. Para otimizar a qualidade da imagem é necessário entender a causa dos artefatos e como podem ser prevenidos. Os artefatos causados por fatores físicos do aparelho ocorrem por ―falhas‖ ou imperfeições no scanner, ou ainda fatores relacionados à aquisição das imagens. Os artefatos causados pelo paciente estão relacionados ao movimento do paciente e à presença de materiais metálicos na área examinada. E os artefatos de reconstrução ocorrem devido a erros nas reconstruções de secções adquiridas. Os principais tipos de artefatos são artefatos de metal, em anel, de movimento e ruído de Poisson. Tendo em vista a importância do conhecimento de todos os tipos de artefatos que poderão estar presentes nas imagens tomográficas, esta revisão de literatura e relato de caso tem por objetivo classificar e dissertar sobre os diferentes tipos de artefatos, bem como apresentar um caso relatando as implicações nas imagens tomográficas.

It is known that fan-beam computed tomography scanners (FBCT) and cone beam computed tomography (CBCT) have different characteristics, however, both have indications and limitations. In dentistry, CBCT is more used, due to lower radiation dose and lower cost of the exam. However, it has disadvantages such as limited range of the X-raysin some apparatus and loss of 3D reconstruction quality. In both types of CT occurs the formation of artifacts in the image. The artifact word is used to define the discrepancy between the numbers in the reconstructed images of the object and its attenuation coefficient. The artifacts may negatively impact the diagnosis simulating pathologic conditions. The origins of the artifacts are diverse; among them can be mentioned physical factors of the apparatus, patient movement and the image reconstruction. To optimize the image quality is necessary to understand the cause of the artifacts and how they can be prevented. The artifacts caused by physical factors of the device occur by "failures" or imperfections on the scanner, or factors related to the acquisition of the images. The artifacts caused by patient are related to patient movement and the presence of metallic materials in the examined area. And the reconstruction artifacts occur due to errors in the reconstruction of acquired sections. The main types of artifacts are metal, ring and Poisson noise. In view of the importance of knowledge of all kinds of artifacts that may be present on CT images, this literature review and case report main to classify and discourse about the different types of artifacts as well as to present a case report with artifacts implications on CT images.
