O papel da Secretaria Estadual de Saúde na implementação dos Comitês de Investigação de Transmissão Vertical de HIV e Sífilis nos municípios de Porto Alegre e Gravataí

Abstract

O presente trabalho se propõe a discutir e analisar todo o processo de implementação dos comitês de Investigação de Transmissão Vertical de HIV/AIDS e Sífilis no estado do Rio Grande do Sul. Como esses comitês atuam no sentido de investigação e intervenção dos casos de Transmissão Vertical e como esses comitês funcionam, buscando saber como se dá a interação com a rede, quais os obstáculos enfrentados em ambas as esferas: estadual e municipal. Para tanto, fomos a campo acompanhar algumas reuniões desses comitês e também conversamos com os gestores municipais e estaduais, a fim de analisar como esse processo acontece na prática e se ele está de acordo com o que as normativas estabelecidas pelo Protocolo de Investigação de Transmissão Vertical. Analisamos esse processo de implementação dos comitês de Investigação de Transmissão Vertical de HIV e Sífilis nos municípios de Gravataí e Porto Alegre nos quais foi possível identificar processos distintos de implementação desses comitês. O Munícipio de Gravataí apresenta características do modelo de implementação Top down, não só as características no processo de execução desse comitê, mas também no processo de funcionamento do mesmo, seguindo o Protocolo de Investigação de Transmissão vertical de HIV e Sífilis instituído pelo Ministério da Saúde em 2014 Já o município de Porto Alegre implementou o comitê de Investigação de Transmissão Vertical das epidemias em 2013, ou seja, Porto Alegre foi o município pioneiro na implementação do comitê, sendo essa uma estratégia da política de AIDS. O problema de Transmissão vertical das epidemias foi identificado pela coordenação da vigilância epidemiológica do município, a estratégia foi implementada pelo gestor da política do município. A partir disso, temos dois processos diferentes de implementação nesses munícipios, sendo o que o estado do Rio Grande do Sul tem um papel fundamental de implementação no município de Gravataí sendo o elo entre a esfera federal e municipal, isto é, a estratégia de implementação do comitê e totalmente verticalizada. No município de Porto Alegre, o estado tem um papel mais colaborativo e auxilia nos casos, porém tem um papel menos expressivo do que nos demais municípios. A partir das analises realizadas nesses municípios, podemos percebemos que, apesar dessa estratégia ainda ser recente no estado, vem sendo potencializada cada vez mais no combate das epidemias, pelos gestores.

This study aims to discuss and analyze the entire implementation process for the Vertical Transmission of HIV/AIDS and Syphilis Investigation Committees in the state of Rio Grande do Sul, Brazil. How these committees step in the investigation and intervention in cases of vertical transmission and how these same committees function, seeking to know how the interaction with the network occurs and what are the obstacles to face in both state and municipal scopes. With that purpose, we followed some Committees’ meetings as well as talked to the state and municipal managers in order to learn how the process takes effect and to remark if it complies with the regulations established by the Vertical Transmission Investigation Protocol. We analyzed this process of implementation of the Vertical Transmission of HIV and Syphilis Research Committees in the municipalities of Gravataí and Porto Alegre, which allowed the identification of distinct implementation processes of these committees. The Municipality of Gravataí presents characteristics of the Top down implementation model, not only the characteristics in the process of execution of this committee, but also in the process of its operation, following the Protocol of Research of Vertical Transmission of HIV and Syphilis instituted by the Ministry of Health In 2014. The municipality of Porto Alegre implemented the Vertical Transmission Research Committee for the epidemics in 2013, that is, Porto Alegre was the pioneer municipality in the implementation of the committee, which is a strategy of AIDS policy The problem of vertical transmission of epidemics was identified by the coordination of epidemiological surveillance of the municipality, the strategy was implemented by the manager of the municipality policy. From this we have two different processes of implementation in these municipalities, being that the state of Rio Grande do Sul has a fundamental role of implementation in the city of Gravataí being the link between the federal and municipal sphere, that is, the strategy of implementation of the Committee and fully verticalized. In the city of Porto Alegre the state has a more collaborative role and helps in the cases, but it has a less expressive role than in the other municipalities. From the analysis carried out in these municipalities we can see that, although this strategy is still recent in the state, it has been increasingly strengthened in the fight against epidemics by managers.
