Criocirurgia na oftalmologia veterinária: revisão de literatura

Abstract

A criocirurgia ou crioterapia é uma modalidade terapêutica que consiste na aplicação de substâncias criogênicas no intuito de promover destruição tecidual seletiva. A técnica é utilizada em medicina humana desde os anos 1800 e atualmente está sendo cada vez mais empregada por médicos veterinários. Em comparação a outras opções de tratamento, é uma técnica bastante segura, relativamente rápida, de fácil execução, eficaz, conservadora e de baixo custo. A criocirurgia demonstra possuir uma grande gama de indicações, podendo ser empregada no tratamento de diversas doenças oculares, incluindo anormalidades ciliares, neoplasias, glaucoma, ceratite pigmentar, descolamento de retina e hidrocistoma apócrino. Entretanto, as metodologias empregadas diferem entre si, não havendo um padrão definido a ser aplicado no tratamento de cada doença. Complicações decorrentes da técnica são possíveis, sendo a maior parte devido ao contato prolongado do criógeno com a superfície tecidual e à ausência de controle sobre a temperatura alcançada. Com o presente estudo objetiva-se, através de revisão sistemática de literatura, esclarecer a forma com que a criocirurgia é utilizada no tratamento das afecções oculares que acometem os animais, descrevendo os artigos já publicados, além de resgatar a fundamentação teórica a respeito da técnica.

Cryosurgery or cryotherapy is a therapeutic modality that consists in the application of cryogenic substances in order to promote selective tissue destruction. The technique has been used in human medicine since the 1800's and is currently being increasingly employed by veterinarians. Compared to other treatment options, it is a fairly safe technique, relatively quick, easy to perform, effective, conservative and low cost. Cryosurgery demonstrates a wide range of indications and may be used in the treatment of various ocular diseases, including ciliary abnormalities, neoplasms, glaucoma, pigmentary keratitis, retinal detachment and apocrine hydrocystoma. However, the methodologies used differ, with no defined standard to be applied in the treatment of each disease. Complications due to the technique are possible, most of them related to prolonged contact of the cryogen with the tissue surface and the lack of control over the temperature reached. This study aims, through a systematic review of the literature, to clarify how cryosurgery is used in the treatment of ocular conditions affecting the animals, describing the articles already published, as well as retrieving the theoretical basis regarding the technique..
