Por uma ética artesanal do trabalho em saúde : o caso das agentes comunitárias de saúde

Abstract

Esta dissertação tem por objetivo promover a criação de imagens possíveis produzidas com Agentes Comunitárias de Saúde (ACS) sobre o trabalho e a condição de ACS. Operando com a pesquisa intervenção, produzimos imagens que trazem à tona alguns dos enunciados que operam na produção de subjetividades no trabalho em saúde desses sujeitos. Adotamos a pesquisa intervenção na forma da intervenção fotográfica, tendo como pressuposto que o enunciado também vai passar pelo olhar. Assim a pesquisa se tornou dispositivo de intervenção imagética, desnaturalizando o olhar e mostrando as condições sob as quais podemos ver. Partindo do imaginado como potência para revelar múltiplos sentidos, tomamos a imagem, e mais especificamente a imagem fotográfica, como dispositivo estratégico que opera no sentido de transformar para conhecer. O trabalho foi desenvolvido na equipes de saúde da família do Rincão 1 e 2, no bairro Belém Velho, do município de Porto Alegre RS, entre os meses de junho de 2012 e fevereiro de 2013, com encontros semanais que transitaram entre o acompanhamento do trabalho das ACS na área, bem como a participação em reuniões de equipe e a eventos que a mesma promoveu. Os conceitos de governamentalidade, acontecimento e disciplina, tomados das obras de Foucault, criaram condições para desnaturalizar as noções de público e privado na saúde, assim como operaram da mesma maneira com o surgimento das ACS no Sistema Único de Saúde (SUS) As imagens mostram que dentro da unidade de saúde as normas disciplinam o trabalho, dividindo o trabalho e os trabalhadores através do regramentos das ações e da delimitação dos espaços de atuação de cada um. No território outras imagens sobre o trabalho se fizeram possíveis, mostrando que o ambiente disciplinar se liga fortemente a proximidade com o resto da equipe. Em relação ao trabalho entendemos que olhar para as agentes como “artífices” possibilita recolocar a responsabilidade de criação de vínculos que recai sobre essas trabalhadoras, coletivizando a tarefa e colocando a equipe (a qual inclui as ACS) como produtora de vínculo. Para forjar esse éthos de “artífice” é preciso que as ACS realizem certas praticas de si que possibilitem a elas refletirem sobre si e sobre o mundo. Utilizamos, também, a imagem da armadura que protege contra a infidelidade da vida, colocada pela paraskéue, como um outro modo de pensar a formação dessas trabalhadoras, que ao mesmo tempo são colocadas como centrais e periféricas na reorganização do modelo de assistência em saúde.

This dissertation aims to promote the creation of possible images produced with Community Health Agents (ACS) on labor and the condition of ACS. Operating with the intervention research, we produced images that bring to light some of the statements that operate in the production of subjectivities in health work of these individuals. We embrace research intervention in the form of photographic intervention, with the assumption that the statement will also pass by the look. So the research became imagery intervention device, by undermining the look and showing the conditions under which we can see. From the imagined as power to reveal multiple senses, we take the picture, and more specifically the photographic image as a strategic device that operates to transform to meet. The work was developed in family health teams of Rincão 1 and 2, in the neighborhood Belém Velho, in the city of Porto Alegre RS, between the months of June 2012 and February 2013, with weekly meetings that passed between the monitoring of work of ACS in the area, as well as participation in team meetings and events that this team promoted. The concepts of governmentality, event and discipline, taken from the works of Foucault, created conditions to deconstruct the notions of public and private health sectors, as well as operated the same way with the onset of ACS in the Unified Health System (SUS). The images show that within the health unit standards governing the work, dividing the work and workers through the ruling of the actions and the delimitation of areas of expertise of each. In the territory other images about the work were made possible by showing that the disciplinary atmosphere binds strongly to proximity to the rest of the team. Regarding the work we understand that looking at agents as "craftsmen" enables replace the responsibility of creating bonds that falls on these workers, collectivizing the task and putting the team (which includes ACS) as a producer of bond. To forge this ethos of "craftsman", is necessary that the ACS perform certain self practices that enable them to reflect on themselves and the world. We also use the image of the armor that protects against infidelity of life posed by paraskeue, as another way of thinking about education of these workers, who are placed at the same time as central and peripheral in the reorganization of health care.
