Próteses parciais fixas em dentes com inserção periodontal reduzida

Abstract

A doença periodontal severa leva a perdas dentais e mobilidade dos dentes remanescentes, e para restaurar a função oral próteses fixas são muitas vezes necessárias e preferíveis pois com esse tipo de reabilitação ocorre uma melhor distribuição de forças para o periodonto, e a prótese funciona como uma esplintagem. Este trabalho teve como objetivo revisar a literatura considerando: a) instalação de prótese parcial fixa em pacientes após tratamento periodontal, que foram acompanhados clinicamente em períodos de 3 a 26 anos; e b) considerações mecânicas sobre próteses parciais fixas, especialmente em dentes com inserção periodontal reduzida. Para realizar a revisão foram consultadas bases de dados como PubMed e Web of Science, Portal de Periódicos da CAPES assim como livros-texto, as palavras chave foram as seguintes: fixed partial dentures, periodontal disease, clinical follow-up, insertion loss, biomechanic, reduced arch. Foi observada grande durabilidade das reabilitações, quando seu design e propriedades mecânicas são adequados e quando são acompanhadas e mantidas sob um rigoroso plano de higiene bucal. Os resultados ressaltaram que a prótese e a periodontia são especialidades intimamente relacionadas e interdependentes, e que um bom planejamento protético passa por um planejamento periodontal correto e vice-versa.

Severe periodontal disease leads to tooth loss and mobility of remaining teeth and to restore oral function fixed partial denture are often necessary and preferable because of this better distribution of forces to the periodontal tissue, so that the prosthesis acts as a splinting. This paper aimed to review the literature considering: a) the installation of fixed partial denture after periodontal treatment in patients who were followed clinically for periods of 3 to 26 years, and b) to revised mechanical considerations on fixed partial denture, especially in teeth with reduced periodontal attachment. For this databases such as PubMed and Web of Science CAPES as well as textbooks were analyzed, the key words were: fixed partial dentures, periodontal disease, clinical follow-up, insertion loss, biomechanic, reduced arch. It was observed a high longevity of rehabilitation when the prostheses design and mechanical properties are adequate by planned and when they are monitored and kept under a strict oral hygiene plan. The results indicated that prosthodontics and periodontics are closely related and interdependent, and that prosthetic good planning involves a right periodontal planning and vice versa.
