dc.description.abstractO objetivo deste estudo in vivo foi avaliar a efetividade da técnica de clareamento dental em consultório utilizando-se diferentes tempos e protocolos de aplicação de um gel clareador à base de peróxido de hidrogênio a 38% (Opalescence Xtra Boost, Ultradent). A sensibilidade dental sentida durante o tratamento e o grau de satisfação dos pacientes com o clareamento também foram avaliados. Para isso selecionaram-se 60 pacientes, os quais foram submetidos à técnica de clareamento dental em consultório, sendo cada uma das hemiarcadas superiores submetidas a um protocolo diferente de aplicação do gel clareador. Os pacientes foram divididos aleatoriamente em 3 grupos (n=20): G45 – em uma das hemiarcadas foram realizadas 3 aplicações de gel de 15 min, com trocas do agente clareador entre cada uma delas (45A), e na outra o peróxido de hidrogênio foi aplicado durante 45 min, de forma contínua, sem troca do gel (45B); G30 – em uma das hemiarcadas foram realizadas 3 aplicações de 15 min, com trocas do gel entre cada uma delas (30A), e na outra o gel foi aplicado durante 30 min , de forma contínua (30B); e G20 – em uma das hemiarcadas realizaram-se 3 aplicações de 15 min, com trocas do gel entre cada uma delas (20A), e na outra o peróxido de hidrogênio foi mantido por 20 min de forma contínua (20B). Em todos os grupos foram realizadas duas sessões de clareamento para cada paciente. A avaliação da cor inicial, bem como a cor obtida após a 1ª e 2ª semanas, 1 mês e após 6 meses de tratamento clareador, foi realizada por meio de dois métodos: avaliação visual, utilizando-se escala de cor Vita Clássica (Vita-Zahnfabrik, Alemanha), e avaliação quantitativa, utilizando-se espectrofotômetro Vita EasyShade (Vita-Zahnfabrik, Alemanha). A avaliação clínica da sensibilidade dental foi realizada em dois momentos: imediatamente após as sessões de clareamento e 7 dias após o término do tratamento. O grau de sensibilidade sentido foi registrado, para cada uma das hemiarcadas, segundo os critérios: nenhuma, leve, moderada ou severa. Os dados obtidos foram analisados com o teste t de Student para amostras pareadas, e os resultados mostraram que existe diferença significativa entre os tratamentos A e B para os grupos 30 e 20, tendo os subgrupos B apresentado menos clareamento. Quando os níveis de sensibilidade foram comparados, o teste de Kruskal-Wallis mostrou que não existe diferença entre os grupos avaliados. Conclui-se que a aplicação contínua do agente clareador por 45 min foi tão efetiva quanto as 3 trocas de 15 min. Os tempos de contato contínuo de 30 min e 20 min mostraram-se menos eficazes do que um tempo de 45 min.pt_BR
dc.description.abstractThe aim of this in vivo study was to evaluate the effectiveness of an inoffice tooth bleaching technique using different application times of a 38% hydrogen peroxide– based bleaching gel (Opalescence Xtra Boost, Ultradent). Additionally, the tooth sensitivity felt by the patients during the treatment and the patients’ satisfaction degree regarding the treatment were assessed as well. Sixty patients were selected. Each maxillary hemi-arch was assigned to a different application time of the bleaching gel. The patients were randomly divided into 3 groups (n=20): G45 – three 15 minutes applications, with gel switches between each application (45A) whereas on the other maxillary hemi-arch, the bleaching gel was applied during 45 minutes, without gel switching (45B). G30 – three 15 minutes applications, with gel switches between each application (30A) whereas, on the other maxillary hemi-arch, the gel was applied during 30 minutes without gel switching (30B). G20 – three 15 minutes applications, with gel switches between each application (20A) whereas, on the other maxillary hemi-arch, the gel was applied during 20 minutes without gel switching 3(20B). Two bleaching appointments were scheduled for each patient from all groups. The baseline color assessment, as well as the assessment of the new color patterns achieved after the first and second weeks of bleaching treatment were performed through two methods: visual evaluation using the Vita Classical shade guide (Vita-Zahnfabrik, Germany) and quantitative evaluation using the Vita EasyShade spectrophotometer. The clinical evaluation of the tooth sensitivity was performed in two distinct moments: immediately after the bleaching appointments and 7 days after the end of the treatment. The sensitivity degree felt by the patients was recorded for each hemi-arch according to the following criteria: none, light, moderate or severe. The obtained data were analyzed by the T-Student statistical test for the paired samples and the results showed that there was a significant difference between the treatments A and B for the groups 30 and 20. The subgroups B showed the lowest bleaching outcome. As for the comparison of the sensitivity degrees, the Kruskal-Wallis test showed that there was no difference between the assessed groups. It was concluded that the continuous 45 minutes application of the bleaching gel was as effective as the three 15 minutes of switching applications. The 20 and 30 minutes continuous application times were less effective.en
