Class III malocclusion is characterized by the distal relationship of the mesiobuccal cusp of the first permanent maxillary first molar relative to the buccal groove of the first mandibular first molar. This malocclusion, skeletally, can be caused by maxillary atresia, excessive mandibular growth, or even the combination of these factors. Conventional facemask therapy has predominantly dentoalveolar effects and patient cooperation may be compromised by the unaesthetic appearance of this device. Correction with intermaxillary elastics associated with skeletal anchorage has a greater skeletal effect, does not present a negative aesthetic effect in the treatment, but imposes a higher financial and biological cost to the patient. In this study we present a retrospective of the most widespread protocols in the early treatment of Class III malocclusion and expose a treatment protocol with a lower aesthetic, biological and financial burden for the patient. In addition, the study evaluates the dentoskeletal and soft tissue effects of the treatment of Class III malocclusion with intermaxillary elastics during the mixed dentition phase. To this end, lateral headfilm was the diagnostic tool used to quantify the dentofacial changes achieved during Class III malocclusion correction. Cephalometric variables were measured with the Dolphin 11.7 premium program at baseline and at the end of treatment. A protocol is suggested, consisting of, briefly, 4 x 2 Straight Wire fixed appliance, maxillary expansion with Hyrax and Class III intermaxillary elastic.en
