O processo de enfermagem (PE) inclui a etapa de diagnósticos de enfermagem (DE) definidos após a análise dos sinais, sintomas e fatores de risco apresentados pelos pacientes. No Hospital de Clínicas de Porto Alegre, este método é utilizado há mais de 30 anos, e atualmente encontra-se informatizado. Todavia, o Serviço de Radiologia ainda não tem acesso a este sistema, o que dificulta a identificação dos DEs. Com a finalidade de qualificar o cuidado aos pacientes que utilizam os serviços de enfermagem radiológica e de melhorar os registros de enfermagem, propôs-se este estudo, que teve como objetivos identificar os DEs para pacientes submetidos à biopsia hepática guiada por ultrassonografia e descrever as respectivas intervenções de enfermagem. Trata-se de um estudo descritivo de corte transversal, realizado no Hospital de Clínicas de Porto Alegre, que teve como população os pacientes submetidos à biópsia hepática guiada por ultrassonografia no período de setembro a novembro de 2011. A amostra compreendeu 97 pacientes, selecionados de acordo com a data de realização do exame. A coleta de dados ocorreu nos prontuários, de forma retrospectiva, com foco nas evoluções de enfermagem pré, trans e pós-biópsia hepática. A pesquisa foi aprovada pela Comissão de Pesquisa da Escola de Enfermagem da Universidade Federal do Rio Grande do Sul e pelo Comitê de Ética em Pesquisa do hospital. Foram analisadas 97 evoluções de enfermagem que evidenciaram os seguintes sinais e sintomas: dor (16,49%), pressão elevada (3,09%), agitação e choro (2,06% cada) e preocupação, insatisfação pelo NPO, tontura, sangramento, bradipnéia, bradicardia e medo do resultado (1,03% cada). Os DEs estabelecidos foram: Risco de sangramento, Dor aguda, Medo, Ansiedade, Conhecimento deficiente, Risco de glicemia instável e Conforto alterado. Com o estabelecimento dos DEs, foram descritas as intervenções de enfermagem conforme as informações contidas no sistema informatizado do HCPA e conforme as intervenções de enfermagem da Nursing Interventions Classifications (NIC). ...
