OBJETIVOS: Investigar as tendências da incidência e da mortalidade por tuberculose (TB) nos estados brasileiros, relacionando-as à situação socioeconômica dos respectivos estados no período de 2001 a 2010. MÉTODOS: Trata-se de um estudo ecológico descritivo, utilizando-se dados secundários referentes à situação atual demográfica e socioeconômica, à incidência e mortalidade por TB, entre 2001 e 2010, e ao Índice de Desenvolvimento Humano (IDH), entre 2001 e 2007, para o Brasil e Unidades da Federação. Os dados foram coletados no Sistema de Informação de Agravos de Notificação, no Sistema de Informações sobre Mortalidade e nos sites do Programa das Nações Unidas para o Desenvolvimento, Instituto Brasileiro de Geografia e Estatística e Banco Central do Brasil. Foram utilizados os programas Excel e SPSS for Windows, 17.0, para tabulação e análise dos dados. RESULTADOS: O estado de Santa Catarina e o Distrito Federal (DF) apresentaram a maior expectativa média de vida (75,8 anos), seguidos do Rio Grande do Sul (75,5 anos). O DF também apresentou a maior média de anos de estudo (9,6) e de PIB per capita (R$ 45.977,59), seguido de São Paulo (8,5 e R$ 24.456,86 respectivamente). No outro extremo, o estado de Alagoas apresentou menores médias de expectativa de vida e anos de estudo, e o Piauí apresentou o menor PIB per capita. Os estados com maior variação na redução da incidência de TB entre os anos de 2001 e 2010 foram Rondônia (51%) e Acre (49,94%). Contudo, o Amazonas apresentou a maior incidência da doença em 2001 (101,9) e o Rio de Janeiro em 2010 (68,1). Já o Distrito Federal apresentou o menor coeficiente de incidência para o período avaliado. Em relação à mortalidade por TB Paraíba apresentou a maior variação no aumento do coeficiente (53,3%) e Roraima, na redução deste (76,3), seguido do Acre (69,2%). O estado do Rio de Janeiro apresentou os coeficientes de mortalidade mais elevados (7 em 2001, e 5,6 em 2010), entre os estados do país, enquanto que o DF e Tocantins apresentaram os menores coeficientes de mortalidade. O Distrito Federal se destacou com o melhor IDH do país (0,858 em 2000, e 0,900 em 2007) e Alagoas com o pior IDH para o período de 2000 (0,648) a 2007 (0,722). Observou-se tendência de redução da incidência e mortalidade por TB, e elevação do IDH no Brasil, para o período observado. Verificou-se uma força de correlação fraca entre os coeficientes de incidência e de mortalidade por TB e o IDH para os anos de 2001, 2004 e 2007. CONCLUSÕES: Observou-se uma redução tanto nos coeficientes de incidência, quanto de mortalidade por TB na maioria dos estados brasileiros, embora com algumas oscilações no período e 2001 a 2010. Houve aumento do IDH em todos os estados entre 2001 e 2007. Considerando-se a importante disparidade nas variáveis observadas entre os estados brasileiros, torna-se fundamental o redimensionamento de políticas públicas voltadas ao controle da TB e da promoção da saúde de acordo com necessidades locais ou regionais. ...
