A obesidade é um dos fatores de risco para o desenvolvimento de infecção pós-cirúrgica, sendo estimado, portanto, uma maior incidência para esses pacientes. A cirurgia bariátrica é um procedimento de alta complexidade, indicado para o tratamento da obesidade mórbida. A profilaxia com antimicrobianos para reduzir essa complicação parece ser uma prática consensual entre os cirurgiões para vários procedimentos. A eficácia da antibióticoprofilaxia requer a seleção adequada dos antimicrobianos de acordo com o tipo de procedimento cirúrgico. Requer ainda utilização de esquema adequado, considerando dose, início de administração e o reforço de dose se necessário para a manutenção dos níveis séricos e teciduais do fármaco durante a cirurgia. A literatura apresenta um número reduzido de trabalhos sobre a antibióticoprofilaxia em cirurgia bariátrica, sendo incluídos na presente revisão quatro estudos, alguns observacionais. Considerando os resultados destes estudos e o conhecimento acumulado sobre antibioticoprofilaxia cirúrgica, o esquema profilático mais adequado para essa operação parece ser a administração de cefalosporina de 1º geração, a cefazolina, na dose de 2g intravenosamente no momento da indução anestésica em administração única. ...

The effectiveness of antibiotic prophylaxis requires the selection of appropriate antimicrobials according to the type of surgical procedure. Obesity is a risk factor for developing postoperative infection, it was estimated, therefore, a higher incidence for these patients. Prophylaxis with antimicrobials to reduce this complication appears to be a consensus among surgeons for many procedures. The effectiveness of antibiotic prophylaxis requires the selection of appropriate antimicrobials according to the type of surgical procedure. Use of appropriate prophylactic schedule must consider dose, the beginning of initial administration and strengthening of dose if necessary for the maintenance of serum and tissue drug concentration during surgery. Bariatric surgery is a highly complex procedure, indicated for the treatment of morbid obesity. The literature presents a limited number of papers on the subject, being accessed four studies on the topic. Considering the results of these studies and the accumulated knowledge about surgical antibiotic prophylaxis, the prophylactic regimen most suitable for this operation seems to be the administration of the first generation cephalosporin, cefazolin in particular at a dose of 2g intravenously at induction of anesthesia in a single administration. ...
