Protocolo para atendimento clínico-cirúrgico de pacientes com diagnóstico de deformidades dentofaciais da unidade de cirurgia buco-maxilo-facial do hospital de clínicas de Porto Alegre.

Abstract

As deformidades dentofaciais são anomalias decorrentes de alterações na formação e no desenvolvimento do complexo buco-maxilo-facial. Podem ter origem adquirida ou congênita, atingindo a mandíbula e a maxila isoladamente, combinadas entre sí ou associando-se aos demais ossos da face. A velocidade, direção e quantidade de crescimento são caracterizadas pela etiologia e individualidade. As deformidades causam no paciente dificuldades na mastigação, deglutição, respiração, fonação além de problemas de convívio social. Diante das dificuldades funcionais do sistema estomatognático, pode-se comprometer o restabelecimento das condições sistêmicas do paciente. Após identificação da etiologia e determinação do grau de discrepância dentofaciais, osteotomias envolvendo a maxila e mandíbula, de forma isolada ou combinada podem ser realizadas; e por meio de avanços, retrusões, rotações, ablações ou enxertos ósseos, resultam na reestruturação tridimensional óssea e muscular da face. O tratamento destas anomalias tem a oclusão dentária como guia para o estabelecimento estrutural do sistema estomatognático. O objetivo deste estudo foi desenvolver um protocolo para o atendimento clínico-cirúrgico de pacientes da Unidade de Cirurgia Buco-maxilo-facial do Hospital de Clínicas de Porto Alegre, com diagnóstico de deformidades dentofaciais. Uma revisão da literatura atual foi realizada acerca desse tópico, através de buscas nas bases de dados Pubmed e Lilacs, assim como em livros teóricos, baseado na qual, após análise crítica das informações coletadas, foi elaboradado um protocolo para uso assistencial nos casos de deformidades dentofaciais.

Dentofacial deformities are anomalies resulted from alterations in the formation and development of the buccomaxillofacial complex. The etiology of these deformities can be acquired or congenital, and reach the jaw or maxilla alone, combined with each other or associated with the other bones of the face. The speed, direction and amount of growth are characterized by etiology and individuality. Those deformities can possibly result in difficulties to chew, swallow, breath, changes in the phonation process, as well as result in social problems. Given the functional difficulties of the stomatognathic system, the restoration of the patient's systemic conditions may be compromised. After identifying the etiology and determining the degree of dentofacial discrepancy, osteotomies involving the maxilla and mandible, alone or combined could be performed; and through advancements, retrusions, rotations, ablations, or bone grafts, result in three-dimensional bone and muscle restructuring of the face. The treatment of these anomalies has dental occlusion as a guide for the structural reestablishment of the stomatognathic system. The aim of this study was the development of a protocol for the clinical and surgical care of patients from the Buccomaxillofacial Surgery Department of the Hospital de Clínicas de Porto Alegre, with diagnosis of dentofacial deformity. A review of the current literature was conducted on this topic through searches in the Pubmed and Lilacs databases, as well as in theoretical books, based on which, after critical analysis of all information collected, a protocol was developed for use of assistance in cases of deformities dentofacial surgery.
