Introdução: A doença renal crônica (DRC) é uma disfunção silenciosa que leva a perda lenta, progressiva e irreversível da função renal. A fisiopatologia da doença, associada ao impacto do tratamento hemodialítico, leva a uma baixa tolerância ao exercício e descondicionamento físico, colaboram para a diminuição da funcionalidade e impactando negativamente a função pulmonar, a capacidade cardiopulmonar e a qualidade de vida destes indivíduos. O nível de atividade física e a relação com a função pulmonar, a capacidade cardiopulmonar, a força muscular e periférica e a qualidade de vida ainda não estão bem elucidadas em pacientes com DRC em tratamento hemodialítico. Objetivo: Determinar o NAF e correlacionar com as variáveis de função cardiopulmonar, espirométricas, de força muscular respiratória e periférica e de qualidade de vida nos pacientes com DRC em tratamento hemodialítico. Metodologia: Estudo transversal com indivíduos estáveis clinicamente, com 18 anos ou mais, diagnosticados com DRC há mais de 6 meses, em tratamento hemodialítico no Serviço de Nefrologia do Hospital de Clínicas de Porto Alegre (HCPA) por pelo menos 3 meses. Todos os sujeitos foram submetidos a ao uso do pedômetro (contagem de passos), ao teste de esforço cardiopulmonar (TECP), a espirometria, a manovacuometria, ao teste de 1RM e ao questionário de qualidade de vida. Resultados: Foram incluídos 35 pacientes nesse estudo; a média de idade foi de 54,3±13,3 anos. A média de passos/dia monitorado pelo pedômetro foi de 5679,7 ± 4881,6 (634,4 - 20422,2). O NAF demonstrou uma correlação moderada com VO2pico (r = 0,57; p = 0,001), com VE ( r = 0,407; p = 0,031), com o VEF1 ( r = 0,436; p = 0,020) e com a CVF ( r = 0,419; p = 0,027) em % do predito e com o KDQOLL no domínio funcionamento físico (r = 0,471; p = 0,15).Na classificação do NAF os pacientes altamente ativos representaram 3,22%, os ativos 16,12%, os pouco ativos 12,90%, os ativos 6,45% e os sedentários 64,5%. Conclusão: Pacientes com DRC em tratamento hemodialítico apresentaram NAF reduzido, com prevalência de 64,5% de sedentarismo. Além disso, pacientes mais ativos apresentaram maior capacidade de exercício, avaliada pelo TECP, melhor função pulmonar e melhor qualidade de vida. ...

The pathophysiology of the disease, associated with the impact of hemodialysis treatment, leads to a low exercise tolerance and physical deconditioning, which contribute to reduced functionality and negatively impact lung function, cardiopulmonary capacity and the quality of life of these individuals. The level of physical activity and its relationship with lung function, cardiopulmonary capacity, muscle and peripheral strength, and quality of life are still not well understood in patients with CKD undergoing hemodialysis. Objective: To determine the PAL and correlate it with cardiopulmonary, spirometric, respiratory and peripheral muscle strength and quality of life variables in patients with CKD undergoing hemodialysis. Methodology: Cross-sectional study with clinically stable individuals, aged 18 years or older, diagnosed with CKD for more than 6 months, undergoing hemodialysis treatment at the Nephrology Service of the Hospital de Clinicas de Porto Alegre (HCPA) for at least 3 months. All subjects underwent the use of a pedometer (step counting), the cardiopulmonary exercise test (CPET), spirometry, manovacuometry, the 1RM test and the quality of life questionnaire. Results: Thirty-five patients were included in this study; the mean age was 54.3±13.3 years. The average number of steps/day monitored by the pedometer was 5679.7 ± 4881.6 (634.4 - 20422.2). The PAL demonstrated a moderate correlation with VO2peak (r = 0.57; p = 0.001), with VE ( r = 0.407; p = 0.031), with FEV1 ( r = 0.436; p = 0.020) and with FVC ( r = 0.419; p = 0.027) in % of predicted and with KDQOLL in the physical functioning domain (r = 0.471; p = 0.15). In the PAL classification, highly active patients represented 3.22%, active 16.12 %, the little active 12.90%, the active 6.45% and the sedentary 64.5%. Conclusion: Patients with CKD undergoing hemodialysis had reduced PAL, with a prevalence of 64.5% of sedentary lifestyle. In addition, more active patients had greater exercise capacity, as assessed by CPET, better lung function and better quality of life. ...
