Motivos de atraso intra-hospitalar para o tratamento de reperfusão do acidente vascular cerebral no Hospital de Clínicas de Porto Alegre

dc.description.abstract | Introdução: Quanto mais rápido o paciente com acidente vascular cerebral isquêmico agudo (AVCI) receber tratamento de reperfusão, maiores serão as chances de um bom desfecho funcional. O monitoramento dos tempos de tratamento do AVC permite identificar os principais motivos do atraso da terapia trombolítica e também planejar ações específicas de melhoria. O objetivo deste estudo foi identificar as principais razões para o atraso da terapia trombolítica em pacientes com AVCI agudo de um centro de AVC público. Métodos: Dados de todos os pacientes que chegaram com AVCI agudo no Departamento de Emergência (DE) e receberam terapia trombolítica de 2019 a 2020 foram avaliados prospectivamente. Foi medido os tempos de cada etapa até o tratamento e os motivos de atraso da chegada ao DE até receber terapia trombolítica (o tempo porta-agulha). Resultados: Um total de 143 pacientes receberam terapia trombolítica durante este período. O tempo porta-agulha mediano foi de 73 [56-93] minutos. Pacientes que chegaram pelo Serviço de Atendimento Móvel de Urgência (SAMU) pré-hospitalar apresentaram menor mediana de tempo porta-agulha (70 vs. 80 minutos). Na análise multivariada, os preditores independentes de tempo porta-agulha > 60 minutos foram: fibrilação atrial (FA) prévia (OR 6,8) e a administração do trombolítico no departamento de emergência (OR 8,9; p<0,0001). A maioria dos pacientes teve mais de um motivo para o atraso do tratamento. Os principais motivos foram: o atraso no início da Tomografia Computadorizada (TC) de crânio (tempo porta-TC), atraso na tomada de decisão após a TC (tempo TC-agulha) e atraso na redução da pressão arterial. Diversas ações foram implementadas durante o período do estudo. O mais importante, que contribuiu para diminuir o tempo porta-agulha, foi iniciar o bolus de alteplase na mesa do tomógrafo (diminuiu a mediana do tempo porta-agulha de 74 para 52, tempo ≤ 60 minutos em 67% dos pacientes tratados na mesa do tomógrafo versus 24% dos pacientes tratados no DE). O tempo TC-agulha diminuiu de 49 para 39 minutos em 2020, em comparação com 2019 (p = 0,02), e o tempo porta-agulha diminuiu de 78 para 66 minutos (p = 0,02), respectivamente. Conclusões: O principal motivo de atraso do tratamento de reperfusão com trombolítico foi na realização da TC de crânio, seguido do atraso na tomada de decisão. A implementação do início do trombolítico na mesa do tomógrafo foi o principal fator que contribuiu para a redução do tempo porta-agulha. O monitoramento contínuo dos tempos de serviço é essencial para melhorar a qualidade do centro de AVC e alcançar melhores desfechos funcionais no paciente. | pt_BR |
dc.description.abstract | Background: The faster the patient with acute ischemic stroke (AIS) receives reperfusion treatment, the greater the odds of a good functional outcome. Monitoring AIS treatment times allows to identify the main reasons for thrombolytic therapy delay and also to plan specific actions for improvement. The aim of this study was to identify the main reasons for thrombolytic therapy delay in AIS patients from a public stroke center. Methods: All AIS consecutive patients that arrived in the emergency department (ED) and received IVT from 2019 to 2020 were included in the study. The treatment times of each stage were measured and the delay reasons from arriving at the ED until receiving thrombolytic therapy, the door-to-needle time (DNT), were assessed. Results: A total of 143 patients received thrombolytic therapy during this period. The median DNT was 73 [56-93] minutes. Patients arriving by Emergency Medical Service (EMS) had lower median DNT (70 vs. 80 minutes). In the multivariable analysis, the independent predictors of DNT> 60 minutes were: previous atrial fibrillation (AF) (OR 6.8) and receiving thrombolysis in the ED (OR 8.9). The majority of patients had more than 1 reason for treatment delay. The main reasons were delay starting the CT scan (Door-to-CT time), delay in the decision-making process after CT scan (CT-to-Needle time) and delay to reduce the blood pressure. Several actions were implemented during the study period. The most important that contributed to decrease the DNT was to start the bolus of tPA in the CT scan table (decreased the median DNT from 74 to 52, DNT ≤ 60 minutes in 67% of patients treated on CT scan table vs. 24% of patients treated in the ED). The CT-to-needle time decreased from 49 to 39 minutes in 2020, compared to 2019 (p = 0.02), and the DNT decreased from 78 to 66 minutes (p = 0.02), respectively. Conclusions: The main treatment delay reason was in performing CT scan, followed by the delay in decision-making process. The implementation of the thrombolytic bolus on the CT scan table was the main factor that contributed to reduce DNT. Continuous monitoring of service times is essential for improving the quality of the stroke center and to achieve better patient functional outcomes. | en |
