Base teórica: Nevos melanocíticos traumatizados podem sofrer alterações clínicas, dermatoscópicas e até histológicas, dificultando sua diferenciação de um melanoma. Embora a queixa de trauma em nevos melanocíticos seja frequente, as alterações dermatoscópicas secundárias a ele não são claramente documentas. Objetivos: O objetivo desse estudo é avaliar as alterações dermatoscópicas de nevos traumatizados por dermoabrasão. Métodos: Estudo quase-experimental. Imagens dermatoscópicas de nevos melanocíticos adquiridos benignos de antes e de quatro semanas após a metade de sua área ter sido submetida a dermoabrasão foram comparadas. Resultados: A amostra foi composta por 50 lesões de 15 pacientes. O padrão homogêneo foi o padrão dermatoscópico mais frequente (52%), seguido pelo reticular (16%), cobblestone (14%) e globular (12%). Após a dermoabrasão, quase metade das lesões (46%) tornou-se dermatoscopicamente assimétrica (p <0,001). Em geral, áreas sem estruturas (27/50 para 37/50, p = 0,001), vasos pontilhados (2/50 para 10/50, p = 0,07) e eritema (4/50 para 8/10, p = 0,042) foram as estruturas dermatoscópicas mais frequentes ocorrendo no lado dermoabrasado. Em relação aos padrões dermatoscópicos globais específicos, as lesões reticulares demonstraram diminuição ou perda da rede pigmentada em todos os casos, bem como surgimento de áreas sem estruturas e da cor branca e esmaecimento da cor predominante. A maioria das lesões globulares/cobblestone manteve os glóbulos e apresentou esmaecimento da cor; áreas sem estruturas, vasos em ponto e eritema foram as novas estruturas mais frequentes. No padrão homogêneo, as áreas sem estruturas foram mantidas em todos os nevos e rede pigmentada, vasos em ponto e branco foram as novas estruturas dermatoscópicas mais frequentes. Conclusões: Nossos dados mostram que o trauma após a dermoabrasão pode induzir alterações dermatoscópicas significativas nos nevos melanocíticos. Embora o padrão global não tenha mudado, a maioria das lesões tornou-se assimétrica, com o aparecimento ou esmaecimento das estruturas dermatoscópicas e das cores. Uma história de trauma recente deve ser questionada ao avaliar lesões pigmentadas com as alterações dermatoscópicas descritas. ...

Background: Melanocytic nevi submitted to trauma can undergo clinical, dermoscopic and even histological changes, making it difficult to differentiate them from a melanoma. Although the complaint of trauma in melanocytic nevi is frequent, dermoscopic changes secondary to trauma are not clearly documented. Objectives: The aim of this study is to evaluate the dermoscopic changes of traumatized nevi after dermabrasion. Methods: Dermoscopic images of acquired benign melanocytic nevi were compared before and 4 weeks after half of their area had undergone dermabrasion. Results: The sample consisted of 50 lesions from 15 patients. The homogeneous pattern was the most frequent dermoscopic pattern (52%), followed by the reticular (16%), cobblestone (14%) and globular (12%). After dermabrasion, almost half of the lesions (46%) became dermatoscopically asymmetric (p <0.001). Among all lesions structureless areas (27/50 to 37/50 cases, p = 0.001), dotted vessels (2/50 to 10/50, p=0.07) and erythema (4/50 to 8/50, p=0.042) were the most frequent new dermoscopic structures occurring on dermabrased sides. Regarding specific global dermoscopic patterns, reticular lesions demonstrated decrease or loss of pigmented network in all cases as well as new occurrence of structureless areas, fading of predominant color and development of white color. Most of globular/cobblestone lesions maintained the globules and showed color fading; structureless areas, dotted vessels and erythema were the most frequent new structures. In the homogeneous pattern, the structureless areas were maintained in all nevi and pigmented network, dotted vessels and white color were the most frequent new dermoscopic structures. Conclusions: Our data shows that trauma after dermabrasion may induce significant dermoscopic changes in melanocytic nevi. Although the global pattern did not change, most of the lesions became asymmetric with the appearance or fading of dermoscopic structures and colors. A history of recent trauma should be questioned when evaluating pigmented lesions with such dermoscopic changes. ...
