No entanto, muitas síndromes demenciais passam despercebidas, pois os médicos não suspeitam e, muitas vezes, não as reconhecem mesmo quando há evidências. A importância do diagnóstico no início dos quadros mostra evidência de melhores benefícios. Objetivo: Foi verificar a freqüência de encaminhamento pelo médico assistente ao ambulatório especializado, após a notificação de um rastreio cognitivo positivo Além de verificar a freqüência de prejuízo cognitivo nos pacientes atendidos nesses ambulatórios através do Mini Exame do Estado Mental (MEEM). Material e Método: A amostra deste estudo foi composta por pacientes que aguardavam atendimento em especialidades médicas do Hospital de Clínicas de Porto Alegre (HCPA), com idade ≥ 65 anos. Durante 4 meses os pacientes foram submetidos a um questionário com dados demográficos e o MEEM. No atendimento de cada paciente do estudo, o médico assistente recebia o escore do MEEM realizado e uma orientação quanto à necessidade ou não encaminhamento para avaliação cognitiva especializada. Resultado: Dos 619 idosos incluídos, 152 (24%) dos pacientes apresentavam rastreio positivo para prejuízo cognitivo através do MEEM. Desses 152 pacientes, somente 50 (33%) foram encaminhados para uma avaliação diagnóstica no ambulatório especializado. Sendo que 15 (27%) preenchiam critérios para demência. Conclusão: Observou-se uma baixa taxa de encaminhamentos, enquanto que a freqüência de positividade foi similar à observada em outros estudos com pacientes da comunidade, mas menor que nos estudos com pacientes ambulatoriais. Os pacientes idosos podem apresentar prejuízo cognitivo facilmente identificado através de uma rápida avaliação cognitiva, reforçando a aplicação nas avaliações rotineiras.
