Rotação interna tibial no diagnóstico clínico de associação de lesão do ligamento anterolateral e do ligamento cruzado anterior

Abstract

A restauração da instabilidade anteroposterior do joelho tem mostrado resultados reprodutíveis com as técnicas atuais de reconstrução do Ligamento Cruzado Anterior, abertas ou por via artroscópica. Entretanto, o controle da instabilidade rotacional tem sido difícil de conseguir, apesar de usar técnicas de reconstrução anatômicas. Portanto, várias técnicas têm surgido com utilização de reforços nas reconstruções intra-articulares (dupla-banda) ou por reconstrução de estruturas na face lateral do joelho. Recentemente, tem reacendido o interesse no Ligamento Anterolateral como sendo este um importante contribuinte no controle rotacional em joelhos com deficiência do Ligamento Cruzado Anterior. Estudos recentes têm surgido para avaliar a anatomia e a biomecânica do Ligamento Anterolateral do joelho, tentando definir sua importância e a necessidade de ser reconstruído, a fim de se obter um melhor controle da instabilidade rotacional. Porém, ainda faltam dados de imagem e testes de exame físico que orientem o cirurgião a definir quais pacientes teriam indicação de submeterem-se à reconstrução do Ligamento Anterolateral, associada à reconstrução do Ligamento Cruzado Anterior. Este estudo aborda uma avaliação laboratorial em cadáveres da correlação da rotação interna tibial com o diagnóstico de lesão do Ligamento Anterolateral e estruturas anterolaterais no controle rotacional em joelhos com insuficiência do Ligamento Cruzado Anterior.

The restoration of anteroposterior instability of the knee has shown reproducible results with the current techniques of Anterior Cruciate Ligament reconstruction, either open or arthroscopic. However, the control of rotational instability has been difficult to achieve, despite using anatomical reconstruction techniques. Therefore, several techniques have arisen with the use of reinforcement in the intra-articular reconstructions (double-bundle) or reconstruction of lateral structures of the knee. Recently, interest in the Anterolateral Ligament has been revived as one being an important contributor in rotational control in knees with Anterior Cruciate Ligament deficiency. Recent studies have emerged to evaluate the anatomy and biomechanics of the Anterolateral Ligament of the knee trying to define its importance, and its need to be reconstructed to obtain a better control of rotational instability, but still lack imaging data and physical examination tests that advise the surgeon to define which patients would be indicated to undergo reconstruction of the Anterolateral Ligament associated with the reconstruction of the Anterior Cruciate Ligament. This study addresses a laboratory evaluation in corpses of the correlation of internal tibial rotation with the diagnosis of Anterolateral Ligament insufficiency and anterolateral structures in rotational control in knees with anterior Cruciate Ligament insufficiency.
