dc.description.abstractIntrodução: Apesar dos avanços na terapia antimicrobiana, cuidados intensivos e técnicas cirúrgicas não houve alteração nos últimos anos da morbimortalidade na sepse ou choque séptico. Dentre os vários mediadores inflamatórios envolvidos na sepse . as citoquinas ocupam lugar chave na patogênese do choque séptico, muitas vezes com efeitos biológicos opostos. como fator de necrose tumoral-alfa e a endotelina-1. Além disso. há um grande interesse nos níveis circulantes destas citoquinas pela possibilidade de identificar pacientes de a lto risco na síndrome séptica. Entretanto. o valor preditivo dos níveis plasmáticos do fator de necrose tumoral-alfa. de seus receptores solúveis e. particularmente da endotelina- 1 não está definido. principalmente quando analisado precocemente no choque séptico. Objetivo: Avaliar a evolução sequencial dos níveis plasmáticos de fator de necrose tumoral-alfa. receptor solúvel-I do fator de necrose tumoral-alfa , receptor solúvel-2 do fator de necrose tumoral-alfa e endotelina-1 imediatamente após a colocação de cateter em artéria pulmonar durante 24 horas, nos intervalos de O. 6, 12 e 24 horas e determinar o valor preditivo dos níveis plasmáticos obtidos destes mediadores em pacientes com choque séptico. Métodos: Foram estudados 23 pacientes com choque séptico definido pelos critérios do American College of Chest Physicians/Critical Care Society no período de 14 de março a 3 I de agosto de 1996, sendo exclui dos dois pacientes. Foram coletadas amostras de sangue em átrio direito através da porção proximal do cateter colocado em artéria pulmonar nos momentos de O. 6. 12 e 24 horas. As amostras foram centrifugadas e armazenadas a menos 20°C. Posteriormente foram analisados, pelo método ELISA. os níveis plasmáticos do fator de necrose tumoral-alfa. dos receptores solúveis- i e 2 do fator de necrose tumoral-alfa .. li c endotelina-1. bem como as características clínicas e hemodinâmicas dos pacientes da amostra. Após os pacientes foram divididos em sobreviventes e não sobreviventes. Resultados: O grupo de não sobreviventes (n=IO) apresentou valores significativamente mais elevados que os sobreviventes (n=ll) para a idade (64,5 ± 16.6 versus 43.7 :t 12.8 anos), APACHE II (27,7 ± 4,9 versus 18,2 ± 3,7), disfunção orgânica múltipla (3,7 ± 0.9 versus 2.7 ± 0.9) e predominância do sexo masculino (7 versus 2). Não houve diferença significativa entre o tempo de choque prévio à entrada no estudo. valores hemodinâmicos e de transporte de oxigênio entre sobreviventes e não sobrevi ventes. Ambos os grupos apresentaram médias mais elevadas de todos os marcadores biológicos estudados comparados aos valores de referência. Entretanto no momento das 6 horas houve aumento significativo nos não sobreviventes em relação aos sobreviventes para o fator de necrose tumoral-alfa. receptor solúvel-i do fator de necrose tumoral-alfa e endotelina-1. sendo o valor preditivo positivo destes mediadores para mortalidade de 70%, 66,7% e 72.7%. respectivamente. Além disto, a presença de fator de necrose tumoral-alfa maior do que 15.5 pg/ml ou endotelina-1 maior do que 27,8 pg/ml, em pacientes com choque séptico, mostrou sensibilidade e valor preditivo negativo de I 00%. Conclusão: Neste estudo caracterizamos o comportamento temporal de vários mediadores biológicos em um período de 24 horas através de medidas sequenciais de 6 em 6 horas entre as coletas. Demonstramos que os níveis destas citoquinas estão aumentados em todos os pacientes com choque séptico, porém com concentrações plasmáticas significativamente mais elevadas em pacientes não sobreviventes, em especial no momento das 6 horas. Além disso, a endotelina-1 parece ser um fiel marcador para pacientes de alto risco para mortalidade em choque séptico, pois não apresenta falsos negativos.pt_BR
dc.description.abstractIntroduction: Despite advances in antimicrobial therapy. intensive care and aggressive surgical approach. no significant improvement is apparent in morbidity and mortality in patients with sepsis or septic shock in late years. Among the various inflammatory mediators involved in the pathogenesis of sepsis, cytokines featuring opposite hemodynamic effects- vasodilation and vasoconstriction, such as tumor necrosis-alpha and more recently endothelin-1 appear to play important roles. Furthermore , there is growing interest to determine the ability of circulating leveis of these cytokines to identify high-risk patients with sepsis syndrome. However, the predictive role of circulating leveis of tumor necrosis factor alpha and its soluble receptors, and particularly those of endothelin-1 remain parcially understood. particularly when analyzed early in the course of septic shock. Objectives: To sequencially assess plasma leveis of tumor necrosis-factor-alpha, soluble receptor I and 2 for tumor necrosis-factor-alpha and endothelin-1 immediately after insertion of pulmonary artery catheter for 24 hours with 6 hours time interval in patients with septic shock and determine the predictive value of the leveis obtained. Methods: Twenty-three patients with septic shock according the criteria of the American Collcge ()f Chest Physicians/Critical Care Society were studied from March 14 to August 31. 1996: two paticnts were subsequentely excluded. Blood samples were collect from the right atrium using the pulmonary artery catheter atO. 6. 12 and 24 hours. Samples were centrifugatcd ar.d storcd at -20a C for subsequent combined analysis by ELISA of tumor nccrosis factor-alpha, soluble receptor 1 and 2 for tumor necrosis factor-alpha and cndothclin- 1. Clinicai and hemodynamic variables were also recorded , and at the end of the study thc patient population was then divided in survivors and non survivors. Results: Non survivors (n=IÜ) had significantly higher values for age (64,5 :t 16,6 versus 43.7 ± 12,8 years), APACHE 11 (27,7±4.9 versus 18,2±3 ,7), multi pie organ dysfunction syndrome 3,7±0.9 versus 2.7±0,9) and male predominance (7 versus 2) IV compared to survi vors ( n= li). There are no significant diffcrences relati v e to hours in shock previous to study entry, hemodynamic values and oxygen delivery and consumption between survi vors and non survi vors. 8oth groups had high leveis of the inflammatory markers studied compared to reference values. Furthermore, at 6 hours there were significant increase in the leveis of tumor necrosis factor-alpha, soluble receptor l for tumor necrosis factor-alpha and endothelin-1 in non survivor patients compared to survivors: this pattem indicated positive predictive value for mortality of 70%, 66,7% and 72.7% for these markers, respectively. Also. tumor necrosis factor-alpha higher than 15,5 pg/ml or endothelin-1 higher than 27,8 pg/ml in patients with septic shock had both sensivity and negative predictive value of 100% for mortality. Conclusions: In this study we characterized the temporal pattem of four different cytokines in patients with septic shock during 24 hours with 6 hours time interval between sample collections. We demonstrated that cytokine leveis were increased in ali patients, but significant higher values were observed in non survivor patients compared to survivors, specially in the early time points (6 hours). Furthermore. we also showed that endothelin-1 appeared to be the most reliable marker for high risk patiens since it had no false negative casesen
