dc.description.abstractO ameloblastoma caracteriza-se por ser uma lesão que integra o grupo dos tumores odontogênicos, sendo considerado pela literatura como um dos mais comuns integrantes deste grupo. Tem origem nos remanescentes epiteliais da embriogênese dentária, sendo assim classificado como tumor de origem epitelial. Clinicamente se apresenta como uma lesão assintomática, de crescimento lento, localmente invasivo, com um desenvolvimento benigno, afetando com maior frequência a região de ramo da mandíbula. O objetivo deste trabalho foi relatar e discutir sobre a patologia do ameloblastoma, a realização da cirurgia para ressecção parcial mandibular após recidiva, com a posterior instalação de prótese customizada e adicionalmente avaliar o comportamento biológico da prótese em longo prazo. O presente estudo relata um caso clínico de recidiva de ameloblastoma em um paciente do sexo masculino, 40 anos de idade, que procurou o atendimento no ambulatório do Hospital de Clínicas de Porto Alegre (HCPA) com drenagem de secreção purulenta extrabucal. Paciente relatou histórico de ameloblastoma em mandíbula e a realização de procedimentos cirúrgicos de ressecção e reconstrução realizados em outra cidade. A equipe de cirurgia buco-maxilo-facial do HCPA primeiramente decidiu pela remoção do osso infectado e necrótico. Entretanto, após quatro anos de proservação do caso, foi diagnosticada recidiva da lesão, e então a equipe optou pelo tratamento cirúrgico, removendo a placa de reconstrução previamente aplicada, realizando ressecção da área do côndilo do lado direito e corpo de mandíbula do lado esquerdo. Foi então instalada uma prótese de articulação temporomandibular personalizada através de prototipagem, em titânio, para ficar justaposta ao restante de osso mandibular saudável. Os fatores etiológicos histopatológicos, tipo de abordagem, manejo e precocidade do diagnóstico são fatores que influenciam no tratamento. A utilização de próteses customizadas é uma boa alternativa em casos de grandes ressecções. dc.description.abstractThe ameloblastoma is characterized by being an injury that integrates the Group of Odontogenic tumors, being considered by the literature as one of the most common members of this group. Originates in the epithelial remnants of embryogenesis, thus classified as tumor of epithelial origin. Clinically presents as an asymptomatic lesion, slow-growing, locally invasive, with a benign development in most cases and most frequently affecting the region of the mandible. The present study reports a clinical case of recurrence of ameloblastoma in a male patient, 38 years of age, he went to the emergency room of the Hospital de Clínicas de Porto Alegre with drainage of purulent secretion extrabucal oral. Patient reported history of ameloblastoma in jaw and resection and reconstruction surgical procedures performed in another city. Oral-maxillofacial surgery team of the HCPA first decided by the removal of the infected bone. However, after four years of follow-up, recurrence of injury was diagnosed, and then the team opted for surgical treatment, removing the reconstruction plate previously applied by performing the resection area right side condyle and the body the left side. It was then installed a custom temporomandibular joint prosthesis in titanium, to be juxtaposed to the rest of mandibular bone healthy. The treatment chosen for this case was surgical with partial mandible resection and the subsequent installation of customized prosthesis of aloplástico material that was obtained through prototyping. The etiological factors, histopathological type of approach, and precocity of diagnosis are factors that influence the treatment. The use of customised prostheses is a good alternative in cases of big resections. In this work, after evaluations carried out for seven months after surgery, the patient has no complaints and with the prosthesis fulfilling function.en
