dc.description.abstractA dor é sempre um evento estressante que acarreta em alterações fisiológicas, psicológicas e comportamentais, principalmente quando acompanhada de uma doença crônica, como o câncer. O câncer tem levado a milhares de mortes de crianças a cada ano, sendo que a grande maioria destas vai a óbito sentindo dores intensas. A morte é sempre um evento doloroso, mas a morte ou a iminência de morte de uma criança é ainda mais difícil de ser aceita. A avaliação e o manejo adequado de dor em pacientes na oncologia pediátrica são estratégias fundamentais para proporcionar melhor qualidade de vida aos pacientes sem possibilidades terapêuticas. O objetivo desta pesquisa foi conhecer e descrever a avaliação e o manejo da dor, realizados pela equipe de enfermagem, e a percepção pelos pacientes e familiares nos cuidados à criança em fase terminal, na Unidade de Oncologia Pediátrica. O estudo caracterizou-se por uma pesquisa de abordagem qualitativa, e os materiais qualitativos submetidos à análise de conteúdo temática com base em Minayo. A coleta de dados foi realizada nos meses de Janeiro a Abril de 2018. Os participantes foram quatorze profissionais de enfermagem, atuantes na Unidade de Oncologia Pediátrica do Serviço de Enfermagem Pediátrica do Hospital de Clínicas de Porto Alegre, RS, Brasil, e um adolescente internado em cuidados paliativos, e seu familiar. Da análise das entrevistas emergiram núcleos de sentido que constituíram o tema central: Exercício do cuidar da dor total da criança e do adolescente na terminalidade, e seus desdobramentos: Sentimentos e percepções dos profissionais de enfermagem perante o paciente pediátrico em cuidados paliativos e terminalidade; Avaliação e manejo da dor realizada pelos profissionais de enfermagem; e, a percepção do adolescente e de seu familiar em cuidados paliativos frente ao manejo da dor. O estudo possibilitou compreender que, diante da facticidade existencial da equipe e da família ao enfrentar a situação de cuidados paliativos e terminalidade de uma criança ou adolescente, questionamentos filosófico-existenciais afloram e permeiam este processo singular de cuidado à dor total. Evidenciou, igualmente, que perante a terminalidade em cuidados paliativos, os profissionais de enfermagem revelam extrema preocupação em tratar a dor de crianças e adolescentes, seja com tratamentos farmacológicos ou estratégias não farmacológicas. O cuidado está intimamente relacionado ao vínculo estabelecido entre o profissional, a família e a criança ou adolescente, o que faz com que o profissional e a família, movidos pela compaixão, centrem seus cuidados não somente no biológico, explicitando uma genuína valorização da vida em sua totalidade.pt
dc.description.abstractPain is always a stressful event which results in physiological, psychological and behavioral changes, especially when is followed by a chronic illness, like cancer. Cancer has led to thousands of children's deaths each year, being the majority of these come to death feeling intense pains. Death is always a painful event, but the death or the eminence of the death of a child is even tougher to be accepted. The proper evaluation and management of the pain in patients in the pediatric oncology are fundamental strategies to provide a better life quality to patients without therapeutics possibilities. The aim of this research was to discover and describe the evaluation and management of the pain, performed by the nursing team, and the perception by the patients and members of the family on the cares to the child in the terminal phase in the Unit of Pediatric Oncology. The study was characterized by a research of qualitative approach, and the qualitative materials submitted to the analysis of thematic content based on Minayo. The collect of the data was carried out in the months from January to April of 2018. The participants were nursing professionals acting in the Unit of Pediatric Oncology, child/teenager admitted under palliative care, and his/her family member. From the analysis of the interviews emerged sense nuclei that constituted the central theme: Exercise of caring for the child and adolescent in the terminal, and its unfolding: nursing professional and their feelings in front of the terminally ill pediatric patient; evaluation and management of the pain carried out by nursing professionals and the perception of the teenager and his/her family member in palliative care facing the management of the pain. The study made it possible to understand that, faced with the existential facticity of the team and the family in facing the situation of palliative care and terminality of a child or adolescent, philosophical-existential questions arise and permeate this singular process of total. It was also evidenced that in the face of terminal care in palliative care, nursing professionals reveal extreme concern in treating children and adolescents' pain, whether with pharmacological treatments or nonpharmacological strategies. Care is closely related to the bond established between the professional, the family and the child or adolescent, which causes the professional and the family, moved by compassion, to focus their care not only on the biological, explaining a genuine appreciation of life in its totality.en
