A síndrome de aspiração meconial é uma das patologias de maior prevalência nas Unidades de Cuidado Intensivo Neonatal, afetando principalmente a pacientes a termo, sem doenças previas evidenciadas no período ante natal, causando principalmente disfunção respiratória, levando ao paciente em múltiplas ocasiões a manobras invasivas de ventilação e com isto aumentando o risco de complicações. A lesão pulmonar induzida pelo mecônio é uma reação inflamatória de importante magnitude e com potencial de dificuldades no desmame ventilatório, evolução para hipertensão pulmonar persistente do recém-nascido e ainda dificuldades respiratórias em longo prazo. Esse Relato de Caso mostra o nível de comprometimento que um paciente pode ter ao aspirar mecônio no momento do nascimento, requerendo suporte ventilatório agressivo e antibioticoterapia, com posterior evolução para taquipneia residual persistente, a qual melhorou apenas após uso de corticoide oral. O uso de corticoide precoce ou tardio no tratamento da lesão pulmonar induzida pelo mecônio é controverso, mas também discutido na literatura, tendo sido apoiado nesse caso levando em consideração o conhecimento da fisiopatologia da lesão. ...

The meconium aspiration syndrome is one of the most prevalent pathologies in Neonatal Intensive Care Units, affecting mainly full-term patients, without previous diseases evidenced in the antenatal period, mainly causing respiratory dysfunction, leading to multiple ventilation's invasive maneuvers and thereby increase the risk of complications. Lung injury induced by meconium is an inflammatory reaction of significant magnitude, with potential for delayed ventilatory weaning, progression to persistent pulmonary hypertension and long-term respiratory distress. This case report shows the level of commitment that a patient may have when aspirating meconium at birth, requiring aggressive ventilatory support and antibiotic therapy, with subsequent evolution to persistent residual tachypnea, which improved only after oral corticoid use. Prescribing early or late corticoid in the treatment of meconium-induced lung injury is controversial, but present in the literature.
