Desinstitucionalização em saúde mental : a experiência de São Lourenço do Sul, RS

Abstract

Esta pesquisa tem por objetivo analisar a institucionalização de um serviço de atenção à saúde mental vinculado a uma proposta de desinstitucionalização. Frente às mudanças nas políticas de saúde mental no Brasil e a todo um movimento que se instalou de questionamento ao modelo hospitalocêntrico e excludente de atenção à loucura - a Reforma Psiquiátrica, e às mudanças mais gerais no sistema de saúde brasileiro, principalmente a transferência para os municípios da gestão e organização de seus sistemas locais de saúde, o estudo enfoca a construção de um serviço, o Centro Comunitário de Saúde Mental de São Lourenço do Sul - RS. As técnicas de investigação foram entrevistas com atores de diferentes instâncias (governantes, agentes e usuários) e observação direta das atividades desenvolvidas no serviço. O olhar para a singularidade busca apreender as especificidades e a forma como os sujeitos respondem às determinações locais e do movimento mais amplo. O surgimento do serviço teve como base um trabalho comunitário, onde a criatividade foi fundamental para que o caminho não fosse o da repetição e da exclusão, mas o da mudança. O trabalho envolvendo diferentes segmentos da comunidade é permeado de conflitos e pouco passível de normatizações, construído/reconstruído na própria prática, com os trabalhadores de saúde mental tendo constantemente os seus papéis questionados e revisados.

The purpose of this research isto analise the institucionalization of a mental health service attached to a deinstitucionalization proposal. Faced to the charges in the mental health politics in Brazil and to a whole movement settled down arguing the centred in the hospital model and excluding of atention to mental health - the Psychiatric Reform, and to the more general changes in the Brazilian Health Care System, mainly transfering to the municipalities the gestion and organization of its Local Health Systems, the study focus on the construction of a service, the Mental Health Community Center of São Lourenço do Sul - RS. For the date collection, interviews with actors in di:fferents instances (governors, agents and usuary) and direct observation of activities developed in the service was the technic used. Sighting the singularity aims to apprehend the specifities and the way how the individuais answer to the local determinations and to the widder movement. The service creation was originated by local and community work, were the creativity was fundamental in order to prevent the repetition and exclusion, but to provide changes. The work involving di:fferents groups of community originating conflicts and has a little normalization, building/rebuilding in the own practice, with the mental health works constantly questioning and revising yours hole.
