dc.description.abstractPênfigo vulgar é uma doença mucocutânea crônica, auto-imune e rara que se caracteriza pela formação de bolhas intra-epiteliais resultantes da perda de adesão entre as células epiteliais pela deposição de auto-anticorpos. Usualmente, acomete indivíduos na faixa etária dos 50 anos de idade e pode exibir predisposição genética. As lesões bucais e cutâneas estão presentes ao mesmo tempo em 90% dos pacientes, além disso, as lesões bucais precedem as de pele em 50% dos casos. Clinicamente, as bolhas rapidamente se rompem deixando úlceras dolorosas com fundo necrótico e halo eritematoso. Porém, a mucosa e a pele normal podem apresentar formação de bolha após uma fricção mecânica (sinal de Nikolsky). As lesões intrabucais podem acometer qualquer área da mucosa, todavia há maior prevalência no palato. Em pele, há manifestação mais freqüente na cabeça, tórax e axilas. O diagnóstico final necessita de biópsia seguida de avaliação histopatológica com observação de acantólise intra-epitelial e células de Tzanck. Imunofluorescência direta do tecido revela presença de IgG ou IgM e componentes do sistema complemento nos espaços intercelulares. Imunofluorescência indireta pode apresentar auto-anticorpos circulantes no soro do paciente. A terapia permanente com corticóides e/ou imunossupressores é a principal abordagem para essas lesões. O prognóstico para pacientes com pênfigo não é previsível, porém essa doença gera grande impacto na qualidade de vida dos pacientes e, em alguns casos, a evolução poder ser fatal. O objetivo deste trabalho é relatar um caso clínico de pênfigo vulgar com manifestação em pele e boca, bem como realizar uma revisão de literatura desta complexa doença.pt_BR
dc.description.abstractPemphigus vulgaris is a chronic mucocutaneous disease, autoimmune and rare, that is characterized by intra-epithelial blistering resulting from loss of adhesion between epithelial cells by deposition of autoantibodies. It usually affects individuals between the ages of 50 years of age and genetic predisposition can display. The oral and cutaneous lesions are present at the same time in 90% of patients, in addition, the oral lesions preceding the skin in 50% of cases. Clinically, the bubbles quickly burst leaving painful ulcers with necrotic base and erythematous halo. However, the normal mucosa and skin may have a bubble after formation of mechanical friction (Nikolsky’s sign). The intra-oral lesions may affect any area of the mucosa, although there is a higher prevalence in the palate. In skin, there are common manifestation in the head, chest and armpits. The final diagnosis requires biopsy followed by histopathologic evaluation with observation of intra-epithelial acantholysis and Tzanck cells. Direct immunofluorescence of the tissue revealed the presence of IgG or IgM and system complement’s components in the intercellular spaces. Indirect immunofluorescence may show circulating autoantibodies in the serum of the patient. Permanent therapy with corticosteroids and/or immunosupressants is the main approach for these lesions. The prognosis for patients with pemphigus is not predictable, but this disease causes major impact on quality of life of patients and in some cases the evolution can be fatal. The objective of this study is to report a case of pemphigus vulgaris with demonstration in skin and mouth, and hold a review of the literature of this complex disease.en
