A disfunção endotelial refere-se à alteração da vasodilatação dependente do endotélio e à má regulação da interação endotélio - células sanguíneas, o que gera uma inflamação localizada e, posteriormente, lesões vasculares graves e trombose. A disfunção endotelial refere-se à alteração da vasodilatação dependente do endotélio e à má regulação da interação endotélio - células sanguíneas, o que gera uma inflamação localizada e, posteriormente, lesões vasculares graves e trombose. Geralmente ocorre antes das manifestações estruturais e a sua avaliação clínica pode servir como preditor de eventos cardiovasculares futuros, sendo um marcador precoce da atividade da doença aterosclerótica. O objetivo do presente manuscrito foi revisar aspectos relacionados à fisiopatologia da disfunção endotelial, os métodos empregados na sua avaliação e a relação com os desfechos cardiovasculares. Trata-se de uma revisão bibliográfica, para a qual foram selecionados artigos publicados em inglês e português na base de dados Pubmed utilizando termos de indexação relacionados ao tema de interesse: disfunção endotelial. A partir desta busca a revisão foi estruturada em quatro partes: 1. Fisiopatologia da disfunção endotelial; 2. Métodos de avaliação da função endotelial; 3. Relação entre Disfunção Endotelial e Desfechos Cardiovasculares; 4. Avaliação da função endotelial na prática clínica. Com este estudo, conclui-se que a aplicabilidade da avaliação da função endotelial na prática clínica carece da padronização de um método que seja prático, de baixo custo e que apresente boa reprodutibilidade, além do estabelecimento de pontos de corte específicos para as diferentes populações. As evidências científicas sugerem que a alteração na função endotelial ocorre bem antes das manifestações clínicas e das alterações vasculares e sua avaliação clínica pode servir como preditor de eventos cardiovasculares. ...
