dc.description.abstractIntrodução: A osteonecrose da arcada ósseo-dentária associada à bisfosfonatos tem se tornado um importante efeito adverso associado ao complexo maxilo-mandibular. Os reais eventos que permeiam o desenvolvimento dessa condição permanecem indeterminados. Objetivo: O presente estudo propõe-se a analisar os efeitos do uso dessa medicação no tecido ósseo alveolar, bem como o desenvolvimento de um modelo de estudo reprodutível utilizando o Ácido Zoledrônico como medicação indutora diante da ausência de doença metabólica. Metodologia: A amostra foi composta por 42 ratos wistar, com 60 dias e peso médio de 250g. Os animais foram divididos em seis grupos (GI, GII, GIII, GIV, GV, GVI), diferindo quanto à administração da medicação; extensão da terapia; realização de extração dentária do primeiro molar superior esquerdo e defeito com broca na coroa dentária do primeiro molar inferior esquerdo. Os animais dos grupos GII e GIV receberam dose de 0,2mg/kg de Ácido Zoledrônico, semanalmente, por via intraperitonial, por três semanas. O grupo GVI recebeu prolongamento da duração da terapia totalizando oito semanas. Os animais pertencentes aos grupos GI; GIII e GV receberam doses equivalentes de soro fisiológico. Os grupos GIII, GIV, GV, GVI foram submetidos a procedimentos para extração dentária no primeiro molar superior esquerdo e defeito com broca na coroa dentária do primeiro molar inferior esquerdo, na terceira semana após o inicio do experimento. Ao término de oito semanas todos os animais foram mortos através de câmara de CO² e realizou-se a necropsia e remoção das mandíbulas e maxilas para análise. Os dados foram avaliados por tomografia computadorizada de feixe cônico quanto aos níveis de cinza da imagem e por análise histológica, por meio da atribuição de escores quanto à presença de infiltrado inflamatório, tipo de infiltrado, vascularização, necrose óssea e reabsorção dentinária. Resultados: A comparação entre os grupos GI e GII quanto às avaliações dos níveis de cinza e densidade óssea, não foram estatisticamente significantes (p>0,05). Houve diferença significante quanto aos níveis de cinza na comparação entre os grupos GV e GVI em maxila e entre GIII e GIV em mandíbula (p<0,05). A avaliação histológica mostrou presença de necrose óssea no grupo GVI e menor presença de remodelamento ósseo nos grupos teste (GIV e GVI) comparado aos controles (GIII e GV). A avaliação por escores não demonstrou associação entre nenhuma das varáveis com os grupos (p>0,05). Conclusão: O Ácido Zoledrônico, isoladamente, retardou o reparo ósseo alveolar após extração dentária, no grupo que realizou terapia continuada por oito semanas (GVI) em comparação ao grupo controle (GV), com diminuição dos níveis de cinza. Além disso, a medicação foi capaz de diminuir a capacidade reacional dos tecidos de suporte dentário, apresentando níveis de cinza mais elevados no grupo teste (GIV) quando comparado ao grupo controle (GIII). Alterações histológicas do padrão de normalidade foram observadas nos ossos maxilares dos grupos teste (GIV e GVI). Portanto, este modelo experimental foi capaz de desenvolver alterações no tecido ósseo alveolar de ratos por meio de terapia por Ácido Zoledrônico.pt_BR
dc.description.abstractIntroduction: Osteonecrosis of the Jaw associated with bisphosphonates has become an important adverse effect associated with the maxillo-mandibular complex. The actual events that underlie the development of this condition remain undetermined. Objective: This study aims to analyze the effects of this medication in the alveolar bone, and the development of a reproducible model study using the Zoledronic acid only absence of metabolic disease. Methods: The sample consisted of 42 Wistar rats, 60 days and an average weight of 250g. The animals were divided into six groups (GI, GII, GIII, GIV, GV, GVI), differing in medication administration; extension of therapy; performing dental extraction of the maxillary left first molar and defect to drill in the first lower left molar in the dental crown. The animals of GII and GIV received a dose of 0.2 mg / kg of Zoledronic Acid, weekly, intraperitoneally, for three weeks. The group received GVI prolonged duration of therapy totaling eight weeks. The animals in groups GI, GII and GV received equivalent doses of saline. The GIII, GIV, GV, GVI underwent procedures for tooth extraction in left upper first molar and drill defect in the crown of the first molar tooth left in the third week after the start of the experiment. At the end of eight weeks all animals were killed by CO ² chamber, performed necropsy and removal of mandibles and jaws for analysis. Data were evaluated by cone beam computed tomography as the gray level image and histological analysis, by assigning scores for the presence of inflammatory infiltrate, type of infiltration, vascularization, bone necrosis and resorption dentine. Results: The comparison between GI and GII as the evaluations of gray levels and bone density were not statistically significant (p>0,05). There was significant difference in the levels of gray in the comparison between groups GV and GVI in maxilla and in GIII and GIV (p <0,05) in mandible. Histological evaluation showed the presence of bone necrosis in group GVI and lower presence of bone remodeling in the test groups (GIV and GVI) compared to controls (GIII and GV). The evaluation scores showed no association between any of the variables with the groups (p> 0,05). Conclusion: Zoledronic acid alone, delayed alveolar bone healing after tooth extraction in the group that underwent therapy continued for eight weeks (GVI) compared to the control group (GV), with decreased levels of gray. In addition, the drug was able to reduce the tissues of tooth support capacity reaction, with gray levels higher in the test group (GIV) when compared to the control group (GIII). Histological changes in the normal pattern were observed in the jaws of the test groups (GIV and GVI). Therefore, this experimental model was able to develop alterations in alveolar bone of rats by means of zoledronic acid therapy.en
